Due to an issue with the thermostat on the GPU, the burner unit overheated the glycol solution causing it to flow, as designed, into the adjacent overflow tank. At this point, due to the volume and temperature of the fluid transferred, it escaped the primary overflow tank and was captured by the secondary containment system consisting of an overpack type drum. Unfortunately, the drain valve at the bottom of the overpack drum was not fully seated allowing a portion of the fluid to migrate to the pad surface. |

The pad is equipped with the standard equipment associated with steady state production. A water truck was collecting produced fluid from the production tanks during the inspection. No other activity is occurring at this pad. I spent approximately 5 minutes at each corner of the well pad and did not detect any malodors. No E&S issues were observed during this inspection. No violations noted. |
