Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2237107 Primary Facility 2012-04-27 Routine/Complete Inspection No Violations Noted Ryan Klemish 4/27/12 pre-operations inspection of Site ID 750656 Trecoske South Well Pad, no violations noted. MBC data entry 1/17/14.
2237193 Primary Facility 2014-01-17 Drilling/Alteration No Violations Noted sevice rig preparing to drill out toe
2250490 Primary Facility 2014-02-25 Drilling/Alteration No Violations Noted
2271455 Primary Facility 2014-05-08 Routine/Complete Inspection No Violations Noted Flowback procedures at the site have been completed relatively recently. Carrizo was in the process of breaking down equipment at the site. Residual waste vacuum trucks were in the process of unloading flowback fluid from frac tanks in the tank battery. Mr. Bonnice stated that the 4H well is not in the production phase of operations. The well has been flowed back. Aboveground flow lines have been installed as a temporary measure to route the gas and fluid from the 1H-3H wells to gas production units and production tanks. Eventually, underground flowlines will be installed at a depth of (4) feet to route the gas and fluid from the wells to the gas production units and production tanks. I observed tears in the liner where the gas buster tanks are stored. Mr. Bonnice and Mr. Sodon informed me the gas buster tank containment system would be repaired as soon as possible.

2358349 Primary Facility 2015-04-06 Routine/Complete Inspection No Violations Noted
2413894 Primary Facility 2015-10-07 Routine/Complete Inspection No Violations Noted Remarks: At time of inspection, arrived on site at 12:30 a.m., the cellar had water at the bottom, the (9x5) vent pipe was open and venting to a tank, there was 1622psi on the production casing and 1570psi on the production line. I took a walk around the site and everything looked OK.
2474936 Primary Facility 2016-03-10 Routine/Complete Inspection No Violations Noted The Department was on site to inspect the Trecoske South Well Site(ESX11-115-0090). The wells associated with the pad are as follows:

115-21081 115-21082 115-21083 115-21084

The landowner was working along the access road to the site clearing brush and trees. The operations did not appear to be affecting the LOD of the well site. At the well site, there was evidence of earthwork from the previous year, but it all appeared stable and mulched.

The remainder of the site was stable and well vegetated with no apparent erosional issues. The last company inspection of the erosion and sedimentation controls of the site was on 3/7/2016.

There were no violations.

If there are any issues please feel free to contact me

2511277 Primary Facility 2016-08-15 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:25p.m., the cellar was about a quarter full of water with no bubbling noticed, the (13x9) valve was open and venting into the atmosphere and no gas was detected, the (9x5) vent pipe was open and venting into a tank. There was 856psi on the production casing and 436psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 13:55p.m.

2544270 Primary Facility 2016-11-03 Site Restoration No Violations Noted The Department was on site to inspect the Trecoske South Well Site(ESX11-115-0090). The wells associated with the pad are as follows:

115-21081 115-21082 115-21083 115-21084

The landowner had previously been working just outside the LOD clearing brush and trees. The work had been mulched and seeded and is now growing well. The well site itself is in good shape, with no erosional issues and all vegetation was at or exceeding the 70% vegetative threshold for perennial growth. All BMPs had been removed, and the site was stable.

I recommend APPROVAL of this NOT.

There were no violations.

If there are any issues please feel free to contact me

2602079 Primary Facility 2017-06-07 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:00a.m.

The cellar had water at the bottom with no bubbling noticed. The (13x9) annulus was open and venting into the atmosphere with no gas detected. The (9x5) annulus vent pipe was open and venting into the production tank. There was 732psi on the production casing and 329psi on the production line. I took a walk around the location and there were no obvious concerns noticed. There was a 500bbl flow back tank on containment and the containment appeared to be in good condition. I left the site at 11:40a.m.

2688978 Primary Facility 2018-01-30 Routine/Complete Inspection No Violations Noted On January 30, 2017 I inspected the Trekoske well pads. The access road was very solid and stable. The cut and fill slopes were well vegetated where not stone lined. The wells were all clearly and properly labeled and the pad surface was solid. The surrounding farm field edges had been previously undergoing enhancements and cleanup by the landowner near the access road, but the work has been completed and is now growing well and stable. There were no erosional issues noted. The wells associated with this site are the

2709577 Primary Facility 2018-03-20 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 5' below grade in the cellar and there was no gas detected. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2811349 Primary Facility 2018-12-06 Routine/Complete Inspection No Violations Noted At time of inspection.
