I observed tears in the liner where the gas buster tanks are stored. Mr. Bonnice and Mr. Sodon informed me the gas buster tank containment system would be repaired as soon as possible.

The landowner was working along the access road to the site clearing brush and trees. The operations did not appear to be affecting the LOD of the well site. At the well site, there was evidence of earthwork from the previous year, but it all appeared stable and mulched.

The remainder of the site was stable and well vegetated with no apparent erosional issues. The last company inspection of the erosion and sedimentation controls of the site was on 3/7/2016.

There were no violations.

2511274 Primary Facility 2016-08-15 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:25p.m., the cellar had water at the bottom with no bubbling noticed, the (13x9) valve was closed, the (9x5) vent pipe was open and venting into a tank. There was 586psi on the production casing and 391psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 13:55p.m.

2544267 Primary Facility 2016-11-03 Site Restoration No Violations Noted The Department was on site to inspect the Trecoske South Well Site(ESX11-115-0090). The wells associated with the pad are as follows:

The landowner had previously been working just outside the LOD clearing brush and trees. The work had been mulched and seeded and is now growing well. The well site itself is in good shape, with no erosional issues and all vegetation was at or exceeding the 70% vegetative threshold for perennial growth. All BMPs had been removed, and the site was stable.

I recommend APPROVAL of this NOT.

2602074 Primary Facility 2017-06-07 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:00a.m.

The cellar was empty. The valve on the (13x9) annulus was closed. The (9x5) annulus vent pipe was open and venting into the production tank. There was 10psi on the production casing and 300psi on the production line. I took a walk around the location and there were no obvious concerns noticed. There was a 500bbl flow back tank on containment and the containment appeared to be in good condition. I left the site at 11:40a.m.

2688971 Primary Facility 2018-01-30 Routine/Complete Inspection No Violations Noted On January 30, 2017 I inspected the Trekoske well pads. The access road was very solid and stable. The cut and fill slopes were well vegetated where not stone lined. The wells were all clearly and properly labeled and the pad surface was solid. The surrounding farm field edges had been previously undergoing enhancements and cleanup by the landowner near the access road, but the work has been completed and is now growing well and stable. The well was in production. The cellar was empty and there was no gas detected. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
