2269071 Primary Facility 2014-04-25 Routine/Complete Inspection No Violations Noted All wells on location have been completed and are in the production phase. I recorded (2) milliSiemens/cm conductivity in water next to the berm in the frac tank containment system and next to the Pad berm of the site. There was one frac tank in the containment system. The containment system was not properly secured and had several areas of compromised integrity. I recorded over the limit conductivity in water inside the prodcuction containemnt system. The highest conductivity reading that can be recorded on my meter is (19.99) milliSiemens/cm. The production tanks had noticeable staining. It appears as if the staining occurred as a result of oveflow or misting of production fluid out of the hatches on top of the tanks. I observed a trail of a crystallization/ whitish precipitate on the ground surface of the Pad near the production containment system.

2420717 Primary Facility 2015-10-29 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 8:45, the cellar had water at the bottom, there was some debris in the cellar, the (9x5) vent pipe was open and venting to a tank, there was 1000psi on the production casing and 1044psi on the production line. I took a walk around the location and everything looked OK.
2462547 Primary Facility 2016-02-29 Routine/Complete Inspection No Violations Noted The Department was on site to inspect the Bush Well Pad, ESX11-115-0118. The wells associated with the pad are as follows:

115-20918 115-20919 115-20910 115-20911 115-20912 115-20913

The site was currently undergoing some modifications to the retention bains and the berm along the side of the pad. The site was stable and well vegetated where the site wasn't under construction and there were no apparent erosional issues. There were no violations.

2516391 Primary Facility 2016-08-26 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 14:40p.m., the cellar had water at the bottom with no bubbling noticed, the (13x9) annulus was open and venting into the atmosphere and no gas was detected, the (9x5) vent pipe was open and venting into a tank. There was 729psi on the production casing and 468psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 15:20p.m.

2606514 Primary Facility 2017-06-20 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:40p.m.

The cellar was about a quarter full of water with no bubbling noticed. The (13x9) annulus was open and venting into the atmosphere with no gas detected. The (9x5) annulus vent pipe was open and venting into the production tank. There was 582psi on the production casing and 254psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:30p.m.

2656161 Primary Facility 2017-11-02 Routine/Complete Inspection No Violations Noted I inspected the Bush well pad on November 1, 2017 for a routine inspection. The access road was solid, stable and well graded. There was active ag ongoing off one side with round bales stored just off the road. The pad surface itself was in good shape as well, with good vegetation on both the topsoil pile and the gentle fill slope. There was some firewood cutting at the edge of the pad, but there was no impact to the pad.

There were no violations noted.

2712998 Primary Facility 2018-03-27 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 4' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2740421 Primary Facility 2018-06-12 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Bush well site. The wells associated with this site are the 115-20918, 115-20919, 115-20920, 115-20921, 115-20922. The wells were properly labeled.

The well site and access road were well graded and stable. There did not appear to be any erosional issues along the access road or on the well pad.

The topsoil pile and the fill slopes were well vegetated, with woody plants as well as grasses. There were no areas of instability noted and the slopes appeared stable.

There was no puddling onsite. There was some firewood and small scale sawmilling occurring on the pad edge but this was not affecting the pad.

There were no erosional issues noted.

There were no violations.

2776045 Primary Facility 2018-09-17 Routine/Complete Inspection No Violations Noted I performed an E&S inspection of the Bush wellsite. The wells were all properly labeled. The well pad and access road were solid and stable.

The wellpad had firewood stored on site at the unused corner of the pad.

There were crews on site and work was being performed on the compressor house

The cut and fill slopes were vegetated and stable. The vegetation was healthy and abundant where the ground was not too rocky.

There were no erosional issues noted at the time of inspection.

There were no violations noted.

2815602 Primary Facility 2018-12-14 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 3' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
