Intermittent...not sure what cause is at this point. Operator will investigate and monitor for gas. Walked site. Observed diesel spilled on containment near large storage tank. Nozzle was laying on pad - not in holster. Also observed drill soap spilled onto containment. Violations cited for not containing drilling fluids in a tank. Not all secondary containment is established at this point. Crews are working to get containment in place. I indicated to Co. Man that all containment should be in place prior to drilling. Also asked to increase free board in open half round being used by Fluid Tech.
2077232 Primary Facility 2012-06-19 Drilling/Alteration No Violations Noted Cemented Surface. did not get returns to surface. Estblished TOC at 90'. Planning on doing top job later this evening. Observed bubbling in two cellars on pad. 2H - has welded cap on conductor - steady stream of bubbles observed coming from below cap. 3H - water level above conductor. observed a belch of bubbles approximately every 15 seconds. Checked cellar of 4H - did not observed bubbling in this cellar (vision limited due to wooden mat over cellar). Did not see any bubbling in 1H cellar, could not inspect 13 3/8" annulus due to casing hanger. REquested Operator to investigate bubbling and monitor. Violations cited on 6/18 for two spills - and noted deficiencies in secondary containment. Crews are still on site working on containment in areas addressed during 6/18 inspection. o
2085091 Primary Facility 2012-07-12 Follow-up Inspection Violation(s) Noted On site to inspect condition of wells as part of water supply investigation. Well has been drilled to KOP. Intermediate set. On site with Eric Rooney (DEP Geologist). At time of inspection - both valves on top of well open to atmosphere. Audible blowing heard out of valves. CGI meter read 100% combustible gas. Workers rigging down equipment in area of well while it is venting. BAsed on our observation this is unsafe venting. Violation cited for unsafe venting when it creates a hazard to the public health and safety. At time of discovery I notified Operator of situation. Operator was unaware and indicated a proper vent line will be installed immediately.
2085108 Primary Facility 2012-07-16 Follow-up Inspection No Violations Noted Follow up on venting of well. Cited violation for unsafe venting on 7/12/12. Well is now vented safely. Installed 2" flow line to hot box and flare stack on edge of containment. Met with representative from Carrizo at time of inspection. Planning on installing Barton chart recorder on well and monitor pressure. Also plan on collecting gas samples for isotopic analysis. DEP will collect gas samples from well also. Violation has been corrected at this time.
2085258 Primary Facility 2012-07-20 Follow-up Inspection Pending On site to collect gas samples and inspect condition of well. Met Eric Rooney(DEP geologist) and Doug S. of Carrizo on site at 10 am. Currently have 2" vent line with chart recorder on 9 5/8". Chart recorder reading @ 1psi and 100 lb gauge not showing any pressure. DEP collected two (2) gas samples from needle valve near chart recorder. Observed bubbling in cellar. Bubbling is coming from 2" port which is venting 9 x 13" annulus. Detected @2% conbustible gas from this port. While on site vac trucks pumped out water from all four cellars which were full from recent rains. Observed some bubbling in cellar of 2H. No other bubbling observed on site. 1H (9 5/8") is currently vented to flare stack. Can see visible vapors on flare stack. Well was shut in for @ 10 mins at one point and saw increase in pressure to 20 lbs in that time according to operator. Mr. Weller of Carrizo indicated later in the day that they are going to be loading the hole with SOBM to reduce/kill gas flow in the next couple of days. In the meantime will continue vent.
2105650 Primary Facility 2012-09-24 Follow-up Inspection No Violations Noted On site to collect isotopic gas samples from 13 3/8" x 9 5/8" annulus. At time of arrival annulus was shut it with 100 lb gauge. Reading was @ 4lbs. Eric Rooney on site at 9 am. Collected isotopic samples...when opened up needle valve...pressure quickly dissipated to 0psi with very little gas venting. Methane reading was collected from isotopic bag...reading was 2% using gas meter.

2454159 Primary Facility 2016-02-29 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:35 p.m., the cellar had water at the bottom, the (13x9) vent pipe was open with a 100psi gauge on it that was reading 0psi with no gas detected, the (9x5) vent pipe was open with a 100psi gauge on it that was reading about 36psi and the valve going to the tank was closed, there was 1152psi on the production casing and 10psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 13:20 p.m.

2544390 Primary Facility 2016-10-13 Site Restoration No Violations Noted The Department was on site to inspect the Well Pad, ESX11-115-0138. The wells associated with the pad are as follows:

The site met the 70% vegetative standard, with decent growth everywhere on site. Recently constructed PCSM basins well constructed and well vegetated. No temp. BMPs on site. The vegetation was healthy and the site was stable. There were no erosional issues at this time.

I recommend APPROVAL of this Notice of Termination.

2544895 Primary Facility 2016-12-13 Routine/Complete Inspection Pending At time of inspection, arrived on site at 10:45a.m., the cellar was about three quarters full of water with no bubbling noticed, the (13x9) annulus was open and set up with a 100psi pressure gauge with 0psi on it, the (9x5) vent pipe was open and venting into the production tank. There was 750psi on the production casing and 458psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

After further review, in 2012 the well was part of a gas migration issue. The inspection results was pending on an inspection report 7/20/12 (2085258). I will contact Carrizo to set up a follow up inspection so the (13x9) vents can be checked for gas. I left the site at 11:25a.m.

2545853 Primary Facility 2016-12-15 Follow-up Inspection No Violations Noted At time of inspection, arrived on site at 13:15p.m., for a follow up inspection to check the (13x9) annulus for gas show. I was accompanied by Jon Sassi (DEP Environmental Specialist). We met with Joe Fowler from Carrizo, Joe closed the ball valve on the (13x9) annulus vent pipe before removing the pressure gauge. He then opened the valve while I used an Altair 5X gas meter to check for the presence of gas, there was no gas detected when the valve was opened. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 13:45p.m.

2632984 Primary Facility 2017-09-05 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water and ice 1' below grade in the cellar and there was no bubbling noticed. The (13x9) annulus was shut in with a 100psi pressure gauge with 0psi on it. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
