The Camp Comfort 1H well is in production. The 4H, 5H, 6H, 7H 8H, 9H and 10H wells have been drilled to total depth. The most recent rilling resumption notice was submitted by Talisman to the Department on 8-27-15 for the Camp Comfort 4H well. The 2H and 3H wells have been plugged. While in the process of surveying the pad surface where the liner was staged, there were several small spots of dark staining indicative of oil based mud observed. The spots were located next to the top of the 5H well cellar, in the west corner of the pad past all of the wells and in between the production tanks containment and wells. During the inspection, Big West Oil Field Services used a skid steer to clean up the impacted areas mentioned above in the inspection report. |

A power steering hose on a water truck on site broke, resulting in a 1/2 gallon release of power steering fluid to containment. It was vacuumed up immediately. Company man Billy Watts reported spill. The second spill occurred on 1/20/2016 at approximately 0840hrs. A hydraulic hose on a man lift broke, releasing approximately 1 gallon of hydraulic fluid. The fluid and soil was immediately scooped up into a 5 gallon bucket. Golder and Associates removed soil. Company man Billy Watts reported spill. Conductivity readings were in the range of (10-14) milliSiemens/cm³ on containment in between a gas buster and an OPSCO USA trailer. Billy Watts of Talisman informed us of a friction reducer (FR) spill of (1) gallon in volume on containment which occurred at 9:30 AM on 2/4/16. The friction reducer spilled to containment was immediately recovered and placed back into the blender tank in preparation to be used for frac operations. Kris Perritt of Talisman informed us of a diesel fuel of one-half gallon on the road leading to this well pad, Beaumont Road. The area where the diesel fuel spilled has been remediated. The impacted area where the fuel was spilled at N-41.84285°; W-076.11749°, is marked off with (2) shovels. Impacted material has been placed in a small plastic garbage container staged on plastic sheeting. |
| 2451988 | Primary Facility | 2016-02-12 | Incident- Response to Accident or Event | No Violations Noted | Sheridan Anderson of Talisman Energy USA contacted The DEP Scranton District Office at 8:24 AM on 2/12/16 to report a spill of (17) barrels of production fluid to secondary containment in the south section of the pad. Mr. Anderson stated that the spill occurred at 5:30 AM on 2/12/16 and has been cleaned up. Halliburton is the Talisman subcontractor on-site in the process of conducting hydraulic fracturing operations. The spill occurred due to a valve left open on a frac tank while Talisman was circulating water in frac tanks and the manifold for the tanks to prevent the water from freezing. An estimated (17) barrels of Production Fluid overflowed the frac tank onto the tank battery secondary containment liner. A tear potentially created by a forklift was discovered in the New Pig liner where the fluid was spilled as a heater hose was moved. The impact to the pad surface beneath the liner is unknown at this point. The hole in the liner has been patched. |

We met with Sheridan Anderson, site company man and Jarrod Hughes, Project Scientist of Woodard and Curran. Hydraulic fracturing operations are in progress. Halliburton is on the 13th stage of the Camp Comfort 7H well, with a (22) stages to be completed according to Mr. Anderson. I used a Hanna Soil Test and Electrical Conductivity & Temperature meter and Oakton Multi-Parameter Tester 35 to field screen water in the sump on the pad surface, near the sump on the pad surface and in water on containment behind the frac tanks in the tank battery. I recorded (12) milliSiemens/cm³ conductivity in the sump containing water, (8.8) milliSiemens/cm³ conductivity on the pad surface in the trench near the sump and (2.2) milliSiemens/cm³ conductivity on the pad surface near the end of the trench, east of the sump and (11) milliSiemens/cm³ on containment. |

We met with Jarrod Hughes, Project Scientist of Woodard and Curran, Gordon Bennett, Talisman site safety supervisor and Dell Gunn, Talisman site company man. Frac operations are in progress. Sump #1 was filled with water. It is apparent that production fluid continues to seep into the trench and flow into this sump. Conductivity in water in the sump was (8) milliSiemens/cm³; (3) milliSiemens/cm³ on the pad surface in the trench next to the sump and (1.3) milliSiemens/cm³ at the end of the trench, east of the sump. Barium in the 2/13/16 and 2/16/16 sample results exceeded the PA statewide health standards. Aluminum, Chloride, Manganese and Iron exceeded the EPA secondary maximum contaminant level. Strontium exceeds the EPA drinking water standards. |

Dell Gunn, company man for Talisman called in a small spill on March 4, 2016 to report a spill on the site. The spill occurred at around 0945 hrs. during fracking of the 10H well. When going to flush, the pressure spiked and the safety/pressure relief valve opened briefly, allowing approximately 5 to 10 gallons of an 80%fresh water - 20%flowback water to fall on containment. There was a vac truck on standby on site, and the spill was vac'd up immediately by Midosa. The Department inspected the site and all of the released water was cleaned up. There was adequate vegetation throughout the site, and there were no erosional issues apparent at this time. There are no violations. |

Conductivity in sump #4 was (600) microSiemens/cm³. The Department noted blue fluid in the metal crate where the TIW valves were housed. The Department requested the operator respond appropriately to remove the fluid. The Company Man explained that they would deploy sorbent pads to remove the fluid. An associate on site explained that they believed the fluid to be paint. |

There was nobody in the foreman's trailer, nor were any personnel observed near the wells. The site was quiet and it is assumed there was flowback going on at this time. With the onset of rain, the Department vacated the site. There was adequate vegetation throughout the site, and there were no erosional issues apparent at this time. There are no violations. Jonathan Sassi and Frank Hoehle were on site to inspect the Camp Comfort well site. The site had some ongoing monitoring and excavation work open due to a release earlier this year. (Photo 1) When checked with a Hanna EC and Soil Probe HI98331, the areas around the excavations showed conductivity as follows: at 41.83790 76.11297 there were readings of 1.7millisiemens (mS), and 1mS. The soil in this area was slightly damp. Downgradient of the previous spill area the soil was dry, with readings (in the vicinity of GPS location 41.83767 76.11309) as follows: 0.4mS, 0.46mS, 0.8mS and 0.64mS. Jack Bolyard (Talisman rep) notified Bob Bisignani (NERO-ER) of a spill which occurred at 10:00 PM on 12/25/2016. The spill was referred to the Scranton office through Alice Adams on 12/27/2016 at 11:30AM. 420 gallons (10 bbls.) of production fluid was released to containment on 12/25/2016 at around 6:30AM. The water in the containment froze and will be removed when the ice melts. Jon Sassi contacted Jack Bolyard from Talisman and Frank Hoehle inspected the site on 12/28/2016 The ground during the 12/28/2016 was mostly frozen, but a few readings with a Hanna EC soil conductivity meter were able to be performed. The access road and pad surface were wet but solid due to recent rains. The cut slope and fill slope were both vegetated where rock was not so abundant it prohibited growth. The vegetation on the fill slope was comprised of woody plants (raspberries), providing an excellent soil holding medium. The site was stable overall and there were no erosional issues noted during the inspection. There were no violations observed. |
