The E&S Inspection log indicated that Paul Hovan conducted a Post Rain Event Inspection on 7/27/12. During this inspection it was noted that there were three areas along the fill slope where rills had formed. The site has been seeded and mulched. The area near the portable lavatory should be reseeded and mulched. In the southwest corner of the well pad, an additional section of filter sock has been added and it has been adjusted to compensate for the 65’ addition to the western side of the well pad.

2724718 Primary Facility 2018-04-30 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Bray well site. The wells associated with this site are as follows: 115-20765, 115-21041, 115-21042, 115-21043, 115-21044, 115-21045, 115-21046, 115-21047, 115-21048, 115-21049. The wells were properly labeled.

The cut and fill slopes were small, well vegetated and surrounded by active agriculture. They were stable. The pad surface and access road were solid and stable. There were no erosional issues noted.
