2757357 Primary Facility 2018-07-11 Drilling/Alteration No Violations Noted At time of inspection arrived on location at 15:30. (4) new 20" conductors, 16" mouse-holes and CellarTech units have been set on pad. Liner and drilling mats have been placed on location in preparation for future drilling activities. (4) new annular vent tanks have been placed at edge of pad with annular vents plumbed from cellars to tanks for all (4) new wells on pad.

- There are also (2) existing wells on pad, the 3H and 4H. Both these wells have been temporarily shut in with night caps installed. Both wells had (2) annular vents, the (13 3/8"x 9 5/8") and (9 5/8"x 5 1/2") vents plumbed to individual vent tanks for each well. Gas checks performed at vent tank openings showed: Tank #1 = 100% LEL, 60% CH4; Tank #2 = 100% LEL, 7% CH4. Well heads were covered with protective steel plates limiting access at time of inspection. No markings were noted on these wells vent tanks to indicate which well was which. Gas checks performed 3' away from vent tanks showed 0% LEL, 0% CH4. Since no health and safety concer

2758427 Primary Facility 2018-07-24 Drilling/Alteration No Violations Noted I arrived at the site at 12:49. The weather conditions are cloudy skies, breezy and humid with rain showers and temperatures in the low to mid 70's as I arrived to the site. The rain had stopped shortly after I arrived. Precision Drilling Rig Super Single # 181 is on the location. The current drilling depth is (666) feet. I met with Will Dean, site company man, consultant of Cabot Oil and Gas and Frank Estes of Cabot Oil and Gas out of the Pittsburgh Office. Mr. Dean provided me with the following details in reference to site activities. "The rig mobilized to the site on July 19th. We are currently drilling the 9H well. The surface casing section of this well will be cemented between (720-800) feet. Thus far, the surface casing on the 11H and 13H wells has been cemented. The next well to be drilled with the surface casing cemented will be the 5H. The 3H and 4H were in production, but are currently shut in. 2807103 Primary Facility 2018-10-16 Routine/Complete Inspection No Violations Noted Jon Sassi (EPS) and I arrived on site at 1304 to conduct a FUI inspection on behalf of the Department.

Upon our arrival, we spoke with Brian Kudrick (EHS Consultant - Cabot) who directed us to the location of the release and accompanied us until his colleagues arrived. Roughly 10 minutes after beginning our inspection Steve Watson, Brody Webster, and Steve Catalfamo (all of whom were representing Cabot) arrived on site.

During our discussion with Cabot, they informed us that the liner used for drilling and well stimulation operations is 40 mil thickness layered in a felt-liner-felt arrangement as seen in photo #4. They informed us that Cabot's SOP is to inspect the liner between drilling and fracing and make any repairs as needed. They then remove any contaminants and lay it back down if it is deemed suitable for reuse. According to the aforementioned Cabot representatives we spoke with on site, 2 roll off dumpsters of material have been excavated so far containing 30 yards each.

2808691 Primary Facility 2018-11-21 Incident- Response to Accident or Event No Violations Noted I arrived at the site at 11:05 in response to a reported spill incident of (10) gallons of diesel fuel. The weather conditions are cloudy skies, breezy and chilly with snow flurries, and temperatures in the low 30's. Steve Watson, Environmental Health and Safety Specialist, and Herb Swiney, completions consultant, both of Cabot Oil and Gas were at the site during the inspection. Keane is the Cabot subcontractor at the site conducting hydraulic fracturing (stimulation) operations for the B Augustine 5H, 9H, 11H and 13H wells. Cabot submitted stimulation notices to the Department on 10/24/18. The comments in the notices indicate that frac operations would begin at 8 AM on 10/25/18. Frac operations are temporarily suspended today, 11/21/18, due to the Thanksgiving holiday according to Mr. Watson. Operations will resume on 11/23/18, Mr. Watson indicated.
