2077613 Primary Facility 2012-06-13 Routine/Complete Inspection No Violations Noted During the investigation, I used an Extech, ExStik (2) pH, Conductivity, Salinty and TDS meter to field screen ponded water inside the containment area where the on-site mobile pretreatment system to filter frac flowback fluid is located. I also screened an area outside the containment area in the NW corner of the Well Pad and the edge of the Well Pad off containment behind the storage tanks. Conductivity, Salinity and TDS readings indicated over the limit, with the limit on the meter being 19.99 milliSiemens/cm. I performed sampling analysis of (2) areas of ponded water behind the storage tanks in the west section of the Well Pad, based on the readings on my meter. I departed from the site at 14:00.
2081472 Primary Facility 2012-06-18 Incident- Response to Accident or Event Violation(s) Noted Arrived at the site at 10:30 AM in response to a production fluid release that occurred at the site on 6-15-12. Carrizo reported the release to the Department at 15:50 on 6-15-12. According to Carizzo, the release occurred sometime between 10:30 and 10:45 on 6-15-12. The release occurred as a result of a pipe fitting being removed from its position on a pressurized line. The line then remained open and exposed to the atmosphere. The estimated volume of production fluid released was determined to be (2) to (3) barrels. The production fluid release impacted the surface of the ground on the Well Pad in a relatively close proximity to the production equipment at the site. The fluid traveled in a vertical direction out of the opened pressurized line and then contacted the surface of the ground on the Well Pad. It then changed direction and flowed off the Well Pad a maximum distance of (10) feet down a slope into a vegetated area next to the water transfer line and gas pipeline right of way.
2089153 Primary Facility 2012-08-06 Administrative/File Review Violation(s) Noted The Department is issuing a Notice of Violation based on the sample results of ponded water on the surface of the ground on the Well Pad and an incident report submitted by Carrizo to the Department via e-mail on 7/26/12. The sample was collected during the 6-13-12 inspection of the Baker West Well Pad. The sample was collected as a result of an over the limit conductivity reading in the ponded water. Please refer to the Baker West 1H, 6-13-12 inspection report for details. DEP Labs submitted the final results on 8/6/12 of the samples collected by the Department on 6/13/12. The results indicated elevated concentrations of the following parameters.

2513784 Primary Facility 2016-08-25 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:30p.m., the cellar had water at the bottom with no bubbling noticed; the (9x5) vent pipe was open and venting into a tank. There was 843psi on the production casing and about 599psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 13:05p.m.

2544474 Primary Facility 2016-10-25 Site Restoration No Violations Noted The Department was on site to inspect the Baker well pad (ESX10-115-0029). The wells associated with the pad are as follows:

The well pad was vegetated in excess of the 70% perennial vegetative standard, the grading was up to par and the temporary BMPs had been removed from the site.

The site was stable and well vegetated with no apparent erosional issues.

2608695 Primary Facility 2017-06-27 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:20p.m.

The cellar was empty. The (9x5) annulus vent pipe was open and venting into the production tank. There was 508psi on the production casing and 432psi on the production line. I noticed staining on the ground near the 3H well, I showed Matt Mercaldo of J P Reilly (contractor for Carrizo) where the stain was and he said he would get it cleaned up. I contacted Frank Hoehle (DEP water quality specialist) to let him know of the staining and he said he would do a follow-up inspection. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:00p.m.

2622508 Primary Facility 2017-08-04 Routine/Complete Inspection No Violations Noted I inspected the Baker West well pad on July 20, 2017. The access road leading in was in good condition, with no debris leaving the site onto the macadam road (Turnpike Road). The well pad itself was solid and stable as well, with no excessive mud or ponding. There was some ponding but it just rained heavily the previous day. The cut slope had vegetation where it was not rock. The fill slopes were stable and solid despite recent heavy rains. All vegetation on site was thick, lush and perennial. There were no erosional issues observed during the inspection.

There were no violations noted.

2715658 Primary Facility 2018-04-04 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was cement between the (20x13) and (13x9) annuli and there was no gas detected. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2816509 Primary Facility 2018-12-17 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 4.5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar with a MSA Altair 5X multi gas detector that was calibrated on 12/3/18. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2851764 Primary Facility 2019-03-11 Routine/Complete Inspection No Violations Noted I inspected the Baker West well site on March 11, 2019. The weather was sunny and about 33 degrees F. The site had no activity at the time of inspection. There was a considerable amount of snow and ice still on site.

The access road and well pad surface were fairly icy but stable and did not have any mud or erosional issues. The wells on the pad surface were all enclosed in carports, with clean gravel surrounding them and legible signs on each.

The infiltration basins and fill slopes were stable, as were the cut slope areas. The cut slope had a rocky appearance without much vegetation due to the nature of the ground, and no erosion was noted.

There was a logging operation noted but it was off the well site LOD. It was not affecting the wellsite.

The containment structure surrounding the production tanks and equipment had sufficient freeboard.

There were no areas of instability noted onsite and no sheen was noted on any water surfaces during the inspection.

There were no violations noted.
