2517575 Primary Facility 2016-09-08 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:40p.m., the cellar had water at the bottom with no bubbling was noticed; the (9x5) vent pipe was open and venting into a tank. There was 862psi on the production casing and 331psi on the production line. I took a walk around the location and noticed what appeared to grease in a water puddle. I contacted Jesse Bonnice of Carrizo to suggest it be cleaned up.

I left the site at 13:15p.m.

The cellar was full of water with no bubbling noticed. The (9x5) annulus vent pipe was open and venting into a small plastic tank. There was 474psi on the production casing and 448psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 10:35a.m.

2622520 Primary Facility 2017-08-04 Routine/Complete Inspection No Violations Noted I inspected the Baker North Well site on July 20, 2017. The site was stable at the time of inspection. The access road leading in had no washing or erosional issues and the pad surface was solid with no excessive ponding or mud issues. The vegetation was well established and healthy looking.

There were no violations noted.

2715665 Primary Facility 2018-04-04 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 3' below grade in the cellar with no bubbling noticed and there was no gas detected from the water level to the top of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2817682 Primary Facility 2018-12-19 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar with a MSA Altair 5X multi gas detector that was calibrated on 12/3/18. The production tanks for this site are located on the Baker pad. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
