2014413 Primary Facility 2011-10-25 Routine/Complete Inspection Pending DEP arrived on site at 11:00 am. The purpose of the inspection was to check the status of the wells and note any activity on site. At the time of inspection, crews were on site installing new plastic secondary containment for upcoming frac. Six wells have been drilled (1H - 6H). All six wells are currently shut in and not vented. Inspection of Baker 2H cellar revealed steady bubbling alongside the 9 5/8" casing. Bubbling was also present @ 6" away from the 9 5/8" casing. Due to the level of fluid in the cellar, the source could not be determined. It is requested by the DEP that the source be indentified. The Operator is to notify the DEP when this work is to be completed. The Operator shall provide a summary of the findings, and a description of what repairs need to be completed. According to Chapter 78 Section 78.86. Defective casing or cementing....the Operator has the option to corect the defect or submit a plan to correct the defect for approval by the DEP within 30 days.

Plastic and mating is down. inspected pump trucks. Catch pans under pistons should be cleaned out. A couple of pump trucks have excess grea/oil on them should be cleaned and disposed of. some spots of purple fluid on containment should be cleaned up. Any spilled petroleum products need to be cleaned up off of containment. Fraccing 2,4, and 6.

2509562 Primary Facility 2016-07-28 Routine/Complete Inspection No Violations Noted The Department was on site to inspect the Baker well pad (ESX10-115-0029). The wells associated with the pad are as follows:

2511740 Primary Facility 2016-08-17 Site Restoration No Violations Noted The Department was on site to conduct a Notice of Termination on the Baker well pad (ESX10-115-0029 (1)). The wells associated with the pad are as follows:

The site was rocky, but where there was enough soil for growth, vegetation was meeting or exceeding the 70% vegetative standard.  There were a few remaining BMPs (silt sock) and these were promptly removed by Carrizo employees.  Off the access road to the Impoundment and Well Site the landowner constructed an earthwork gun range, well vegetated and stable.  This is causing no impact to the site or access road.  
The area was stable and had no erosional issues at this time and it is recommended that this Notice of Termination be APPROVED.

2512477 Primary Facility 2016-08-22 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:00p.m., the cellar had water at the bottom with no bubbling noticed; the (9x5) vent pipe was open and venting into a tank. There was 496psi on the production casing and 936psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 13:40p.m.

2606521 Primary Facility 2017-06-21 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:35a.m.

The cellar was about a quarter full of water with no bubbling noticed. The (9x5) annulus vent pipe was open and venting into the production tank. There was 502psi on the production casing and 458psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:20a.m.

2713022 Primary Facility 2018-03-28 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 3.5' below grade in the cellar with no bubbling noticed and no gas detected in the remainder of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2817025 Primary Facility 2018-12-18 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 4' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar with a MSA Altair 5X multi gas detector that was calibrated on 12/3/18. The permit number for the 4H on the entrance sign was incorrect (see photo #2). It was 155-20468 and should be 115-20468. I contacted Jesse Bonnice of BKV to have it corrected. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2851749 Primary Facility 2019-03-11 Routine/Complete Inspection No Violations Noted I performed a Routine Complete (RTNC) inspection of the Baker Well Pad on March 11, 2019. The weather was sunny and relatively warm, around 38 degrees.

The access road was a bit muddy but not unstable. The access road is quite long, passing some small logging operations, as well as a target shooting range situated along its length.

The well pad surface was plowed and mostly wet but stable. The wellheads were encased in a carport structure. They were surrounded by clean gravel and were properly labeled.

The containment surrounding the production tanks and apparatus was mostly empty and had sufficient freeboard.

The cut and fill slopes of the site were rocky and stable. Where there was sufficient soil, there was adequate vegetation. No areas of instability were noted.

The impoundment on site was full to capacity. It would be recommended to draw down the impoundment to prevent overflow and possible resulting E&S issues.
