2113651 Primary Facility 2012-11-06 Routine/Partial Inspection Pending During the inspection it was noted that the site appeared to have reached >70% perennial vegetative cover. No E&S controls were found to be installed during the inspection. The plan maps reflect that 18” silt fence was to be installed however the fence was redlined as of 3/17/2010 by “JHF” and18” composite sock was to replace the fence. An additional section of sock was also redlined into the plan in the north side of the well site between the pad and the access road. It was noted on the plan map the the filter sock was removed as of 11/2/2010 with “JR” initials. Geotextile material was noted to be in the area of the removed filter sock. In the course of the inspection, it was noted that areas to the west of the well pad between the pad and the access road had wetlands flagged. The flagged areas do not appear to be detailed on the 11/12/2009 plan maps. Vegetation commonly associated with wetlands was noted within feet of the fill slope.Additionally, it appears that rock and timber were situated in this area.

2450168 Primary Facility 2016-02-17 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:30 a.m., the cellar was full of water and ice with no bubbling noticed, the (13x9) vent pipe was open and set up with a pop valve with 0psi on it and the valve going to the tank was closed, the was no vent pipe on the (9x5). There was 0psi on the production casing and 900psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 10:10 a.m.

2736887 Primary Facility 2018-05-25 Routine/Complete Inspection No Violations Noted The Department arrived on site at 1230 hrs to conduct a RTNC inspection. The Department was represented by Frank Hoehle (WQS-DEP) and myself.

Proper signage was displayed at the entrance to the access road. The access road was mostly solid and stable. However, several areas containing minor erosion were present on the access road, which the Department recommends be addressed promptly. Water bars were also in place which were slightly deteriorated but functional.

While examining the production fluid tanks on site, the Department noticed that one of the tanks displayed significant corrosion, including pitting, and heavy staining. The Department requests that the deteriorated tank be examined and either replaced or repaired. The Department noticed that a new empty tank was present on site, most likely for this purpose.

The cut slope and fill slope were both well-vegetated and stable with no apparent erosional issues. The pad surface was also solid and stable.
