I encountered Steve Watson, OGI, leaving the site on the access road as I was mobilizing to the site. He informed me of a brine spill that occurred at the site on 2/4/13. Upon arrival on the Well Pad, I met with Brody Webster and Randi Matthew of Cabot and Charles Minturn and Mallory Santoro of Civil and Environmental Consultants Incorporated. Mr. Webster informed me that the spill occurred at 20:30 on 2/4/13. The spill occurred as result of a production fluid tank falling off a front end loader as the tank was being mobilized off-site as Cabot was in preparation to remove the mud shelf section of the pit. Production fluid leaked out of the tank and impacted the surface of the ground on the Well Pad. The quantity of the release was determined to be one barrel, (42) gallons. |

I observed dark gray material and dark staining primarily in the eastern section of the pit, NE corner of the Well Pad. The surface of the ground appeared darker in color from the center of the mud shelf to the eastern section. Compromised felt and liner integrity were observed, in particular where the highest concentration of dark gray material and staining was observed. Also, hay was observed on the ground surface at the bottom of the mud shelf. Apparently, hay has been utilized as a sub base below the felt underlying the liner. |

As the overlying felt layer of the liner was pulled back in several areas in preparation for disposal, dark gray material and dark staining was observed mainly on top of the liner. Some small punctures and perforations were observed in the liner as well. The liner and underlying felt layer were not pulled out of position to determine if the ground below was impacted. This phase of the operation is most likely to take place tomorrow, 2-7-13. |
