A bond log was completed on the surface casing. During installation of surface casing, there were no returns of cement to surface. Operator pumped 30% excess downhole. An additional 120 sks of grout was pumped on the backside of the surface casing. According to Operator, the bond log showed a good bond to depth. A copy of the bond log was provided to DEP. Bond log was completed approximately 8 hours after cementing.

2138004 Primary Facility 2013-02-05 Follow-up Inspection Violation(s) & Outstanding Violations Arrived at the site at 9:20 AM. I encountered Steve Watson, OGI, leaving the site on the access road as I was mobilizing to the site. He informed me of a brine spill that occurred at the site on 2/4/13. Upon arrival on the Well Pad, I met with Brody Webster and Randi Matthew of Cabot and Charles Minturn and Mallory Santoro of Civil and Environmental Consultants Incorporated. Mr. Webster informed me that the spill occurred at 20:30 on 2/4/13. The spill occurred as result of a production fluid tank falling off a front end loader as the tank was being mobilized off-site as Cabot was in preparation to remove the mud shelf section of the pit. Production fluid leaked out of the tank and impacted the surface of the ground on the Well Pad. The quantity of the release was determined to be one barrel, (42) gallons.
2138579 Primary Facility 2013-02-07 Follow-up Inspection Violation(s) & Outstanding Violations Between 12:05 and 12:30 the remaining section of the felt layers and the liner in the mud shelf were pulled back to examine the condition of the ground below. I observed dark gray material and dark staining primarily in the eastern section of the pit, NE corner of the Well Pad. The surface of the ground appeared darker in color from the center of the mud shelf to the eastern section. Compromised felt and liner integrity were observed, in particular where the highest concentration of dark gray material and staining was observed. Also, hay was observed on the ground surface at the bottom of the mud shelf. Apparently, hay has been utilized as a sub base below the felt underlying the liner.
2140765 Primary Facility 2013-02-06 Follow-up Inspection Outstanding Violations - No Viols Req'd Throughout the course of the day, Cabot continued the process of removing the contents of the mud shelf section of the drilling pit. The depth of the mud shelf is believed to be (4) feet. The top (3) feet of the material in the mud shelf has been mostly removed and the excavation has reached to the eastern section of the shelf, close to the wooden stakes used to delineate the boundary of the shelf in the NE corner of the Well Pad. As the overlying felt layer of the liner was pulled back in several areas in preparation for disposal, dark gray material and dark staining was observed mainly on top of the liner. Some small punctures and perforations were observed in the liner as well. The liner and underlying felt layer were not pulled out of position to determine if the ground below was impacted. This phase of the operation is most likely to take place tomorrow, 2-7-13.

2439612 Primary Facility 2016-01-07 Site Restoration No Violations Noted Inspection conducted of the COG Hoover D P1 Well Pad covered under E&S permit # 58-09-802b20. Inspection conducted on 1/7/16. The inspection was conducted to confirm COG’s assessment that the site meets the minimum requirements as defined in Chapter 102.22(C) for permanent stabilization which would thus fulfill the physical requirements necessary for the permits termination. The Department’s determination is that the entire site is in fact permanently stabilized and meets the criteria necessary for the permit’s termination. I recommend this notice of termination be approved.

I took a walk around the location and everything looked OK.
