1855913 Primary Facility 2010-01-08 Drilling/Alteration No Violations Noted Drill rig on site is Patterson Rig 252. Drilling depth is at TD-9760¿. At the time of inspection the crew was thawing out frozen mud lines in order to resume drilling and circulation. The BOP stack was temporarily off at the time of inspection and a bridge plug had been set at 1050¿. Company Man indicated that 5-1/2¿ casing will be set to TD and cemented back up to a depth of 2000¿. The site appeared to be free of any spilled fluid. Plastic secondary containment is in place under the substructure and other equipment on site.
1860503 Primary Facility 2010-01-22 Follow-up Inspection Viol(s) Noted & Immediately Corrected This inspection was performed to follow up on a reported spill at the Site. Upon arrival I met with Company Man Scott Cheeseman. Mr. Cheeseman reported that approximately 1 to 2 bbls of a pre-flush liquid solution composed of water and Ultr Flush II Concentrate, spilled when a hose failed on a batch mixer during cementing of production string. BJ Services was the cementing subcontractor on site at the time the spill occurred. The incident occurred at approximately 12:30 a.m. on 1/22/10. At the time the hose failure occurred, the primary pump was immediately shut down, stopping the flow of pre-flush. It was indicated that the majority of the liquid was contained on plastic, and less than 1 bbl spilled beyond the plastic lining and onto the ground. None of the fluid spilled off the well pad. A copy of the MSDS of the pre-flush material was provided to DEP. The spilled liquid immediately contained and cleaned up using an on-site rig vacuum.

The Department was on site at the A Hunsinger 2&4 Well Site to conduct a Routine Partial Inspection. At the time of the inspection, the Department noted that the E&S Inspection log indicated that Sam Wiernusz (TEC) completed a Post Rain Event Inspection. The well site is in an agriculture area. There is fencing around the perimeter of the well pad and the area outside of the fence is being used as pasture. There are two tracks from a vehicle running alongside the fence. It is presumed to be from the landowner. Also, the areas outside of the fence that do not meet temporary stabilization requirements appear to be an active agricultural area.

2496257 Primary Facility 2016-06-22 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:50a.m., the cellar was about half full of water with no bubbling noticed, the (9x5) vent pipe was open and venting to a tank, there was 534psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 10:35a.m.

2528821 Primary Facility 2016-10-06 Site Restoration No Violations Noted Inspection of the Hunsinger A P1 Well Pad in response to a submitted Notice of Termination request for the Erosion and Sedimentation Control Permit.

The site has been stabilized and vegetation was found to be greater than 70% uniform perennial growth. Temporary E&S BMPs have been removed from the site. No E&S problems were observed at the time of the inspection. Post Construction Stormwater Management BMPs appear to be installed properly according to the site restoration plans. The well pad is less than or equal to the size indicated on the site restoration plans.

Based on the site inspection, I recommend this notice of termination be APPROVED.
