2368862 Primary Facility 2015-04-28 Incident- Response to Accident or Event No Violations Noted At 09:02, I met with Zachary Subra, Randi Mathew, and JJ Magyar (all Range Resources) on the Laurel Hill 1H well pad. The release of produced water had sprayed from the production tank southeast onto the pad. The spill was cleaned up except for the area immediately beneath the stairs. The stairs for the production tank were moved and impacted material was removed from underneath the stairs. The highest field conductivity noted was 580 umhos/cm. Violations Noted and Immediately Corrected.
2503883 Primary Facility 2016-07-20 Routine/Complete Inspection No Violations Noted Routine Inspection; 7/20/2016.

Jared Rutt (DEP) and I entered the site at approximately 08:45 and walked the site. At the time of the inspection, the Sidewinder 61 was onsite drilling the Laurel Hill 25H (081-21577). The containment on the west side of the drill rig near the cuttings mixing/unloading area had very little berm and appeared to be inadequate. The berm around the drilling additives containment had holes and was not effective. I asked the company man to move the drilling additives onto containment or fix the containment where the additives are currently being stored. The company man and Range representatives worked to fix the containment and on 7/21/2016 I received photos of the containment after it was repaired (attached). In addition, Sean VanFleet inspected the site on 7/21/2016 and reported the containment was in the process of being improved.

Currently, the site has not been restored and the ESCGP is expired. I recommend re-permitting the site with a new ESCGP-2 which contains Post
