Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2292750 Primary Facility 2014-07-27 Drilling/Alteration No Violations Noted
2302522 Primary Facility 2014-08-26 Routine/Complete Inspection No Violations Noted Routine Inspection; 8/26/2014.
2315065 Primary Facility 2014-10-14 Preoperation Inspection No Violations Noted Growth along the access road was substantial but spotty. It was obvious a second round of hydro-seed was applied to areas of sparse growth. Roadside ditches and the road surface itself were in excellent condition. Standing water was noted in both basins. I recommend monitoring these basins outside the growing season and acting if needed to promote effectiveness during the next growing season. The pad area has some vegetative growth beginning and the entire area has been hydro-seeded.
2347385 Primary Facility 2015-02-26 Routine/Complete Inspection No Violations Noted Routine Inspection, Range Resources Cornwall B Unit pad; 2/26/2015.
2355215 Primary Facility 2015-03-26 Routine/Complete Inspection No Violations Noted
2366707 Primary Facility 2015-04-24 Drilling/Alteration No Violations Noted Sunny ~39 degrees F.
2378141 Primary Facility 2014-06-09 Incident- Response to Accident or Event Violation(s) Noted Spill Response inspection; 6/9/2015. At 12:00, Zachary Subra (Range) reported the release of an unknown volume of brine/reclaimed produced water on the well pad. At 12:50, I arrived onsite and met with Mr. Subra. Baker Hughes was onsite preparing to stimulate the Cornwall North Unit 4H (081-21481). Mr. Subra pointed out the containment was not sealed to the well cellars. Evidence suggests the cellars filled after heavy rainfall and overtopped. Water from the overtopped cellars appears to have traveled under the containment and was noticed on the west side of the well pad just off containment. Just off containment on the west side of the pad, field conductivity was between 16,000-18,000 umhos/cm. Violations cited to the 4H well (081-21481)
2379302 Primary Facility 2015-06-16 Routine/Complete Inspection No Violations Noted
2381547 Primary Facility 2015-06-19 Incident- Response to Accident or Event Violation(s) Noted The release was caused when heavy rain washed brine off containment liner which had the berms removed to allow equipment to be removed from the site. At 13:35, I arrived onsite and met with Mr. Subra. Much of the pad previously impacted by a release on 6/9/2015 was excavated to allow for the installation of the flow lines from the well heads to the production units and produced water tanks. Since the last inspection, only a small amount of containment remains around the well heads and the adjacent flowback tanks. The remaining containment was cut-back toward the west where the blender and sand equipment was located. It was from this smaller cut-back containment that the most recent release originated. An intercept trench was installed along the west side of the remaining containment and conductivity ranged from 2,500-2,700 umhos/cm while a nearby puddle was 5,250 umhos/cm. Puddles south of this containment were approximately 650 umhos/cm and water ponded around the well heads and flow lines ranged from 1,200-1675 umhos/cm.
2451703 Primary Facility 2016-02-08 Administrative/File Review No Violations Noted The Well Site Restoration Reports submitted to the Department for the Cornwall North Unit 1H (081-21478), Cornwall North Unit 2H (081-21479), Cornwall North Unit 3H (081-21480),and Cornwall North Unit 4H (081-21481), contain no waste disposal information and are considered incomplete. I recommend evaluating the PCSM (Post Construction Stormwater Management) requirements and re-submitting new Well Site Restoration Reports in the future. No Violations Noted.
2626571 Primary Facility 2017-08-09 Routine/Complete Inspection No Violations Noted Routine inspection of unconventional well in production. No violations noted at time of inspection.
2775048 Primary Facility 2018-09-14 Routine/Complete Inspection No Violations Noted Fours wells staged for production phase. Four each water separators. Dump lines wrapped in insulating blanket. No leaks detected from production equipment. No violations noted.
