2452202 Primary Facility 2016-02-11 Routine/Complete Inspection No Violations Noted At the time of the inspection, the site was covered with snow therefore, the Department is unable to verify compliance with 25 Pa Code §102.22, which requires sites to be permanently stabilized with a minimum of 70 percent vegetative cover, with a density capable of resisting accelerated erosion and sedimentation. Furthermore, the well pad has not been restored. Please note the following statement "THIS IS A WELL PAD SITE. UPON THE ABANDONING AND CAPPING OF THE WELLS, THIS PORTION OF THE SITE WILL BE RESTORED TO ORIGINAL CONTOURS" at the bottom of the PCSM plan sheet does not change the requirement under Section 3216 (c) of the Oil and Gas Act which requires that "within nine months after completion of drilling of a well, the owner or operator shall restore the well site." The PCSM Plans for the well site indicate the well will be returned to original contours. I recommend modifying the plans to comply with the restoration requirements or return to original contours and permanently stabilize as permitte
