The 3H well is being fracked at this time. Only three stages remain for the 3H. The entire frack should be completed by today (3/1/2017).

WHEN: 2/28/2017 16:30 WHAT HAPPENED: "Farrell is reporting a small wet area (1.5 x1’) at the edge of the wellpad, within the LOD where gravel ends. Wet area was noted during routine inspection. Handheld meter indicates 10,000 ppm TDS. The well is currently being fracked. There is no flow from the wet area."

RESPONSE IN PROGRESS/WHEN/WHO: "Chief has mobilized a vac truck and is on standby to remove any free liquid. MR Dirt is also on scene to dig a small trench to help determine the source of the liquid and provide a sump for removal."

The DEP arrived on location and met with a Chief employee to have him show the location of highly conductive water. The conductive water is located on western side of the pad surface and off the pad surface ~5 feet into the grass. An interception trench (see photo 2 an

2568888 Primary Facility 2017-03-06 Follow-up Inspection No Violations Noted Two representatives from Chief Oil and Gas were on location during the DEP's inspection. Woodward and Curran (environmental consultant) is currently at the site doing a detailed site profile to determine the extent of the release. A low spot on the pad surface was found to have a conductivity of around 6.8 mili-Siemens/cm. This location is thought to be the location of the release (See photo 3 and 5). An interception trench (See photo 1)was installed below the pad surface in the area where the release is believed to have migrated. No water was observed in this trench. The production water containment and tank were being removed due to concerns of the release being under both. The amount of brine water released could not be determined at this time. The DEP will make a determination at a future time once the operator and consultant determine the extent of the released material. No samples collected at this time.

There were 3 producing wells. Where possible, annuli were monitored using an Altair 5X gas meter calibrated on October 25, 2017.

3H: top gauge: 1500 psi, (0-5000 psi, analog) 13 3/8 x 9 5/8: 0% combustible gas. 9 5/8 x 5 1/2: not accessible. Annuli for 3H & 2H wells were plumbed to a 500 gallon poly tank.

2H: top gauge: 900 psi (0-5000 psi, analog)

13 3/8 x 9 5/8: not accessible. 9 5/8 x 5 1/2: 100% combustible gas. 1H: top gauge: 1200 psi [0 to 5000 psi, analog] 13 3/8 x 9 5/8: not accessible. 9 5/8 x 5 1/2: 20% combustible gas. No violations were observed at the time of this inspection.

2702030 Primary Facility 2018-03-06 Routine/Complete Inspection No Violations Noted A Spill of 5 gallons of flowback made it to the pad surface on 3/2/2018. The release was reported to the DEP on the same day. Two brine storage baker tanks on location for clean-out maintenance on the wells. One already full baker tank was mistakingly being filled by a truck which resulted in the tank being overfilled. The overflow exited the tank and blew beyond the containment. All contaminates removed and placed on plastic liner. Material to be disposed of at landfill. No spilled material discovered during the DEP's inspection. All material appears to be cleaned up. No samples collected. Violations immediately corrected.
