2503077 Primary Facility 2016-07-18 Routine/Complete Inspection No Violations Noted On July 18, 2016 at 1145 hrs., the Department conducted a routine inspection at the Talisman Energy USA Inc.(Talisman), Ugliuzza (05-006) well site in Pike Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Talisman did not have a representative present during the inspection.

The site sign, E&S plan, and weekly inspection reports were on site and available for review. The slopes of the well pad and the topsoil stockpile mostly had good vegetative cover. A few areas on the fill slope were observed to have dead vegetation. A couple of small erosion gullies were forming in these areas. The Department recommends reestablishing vegetation in these areas. Two bio-retention basins were observed to be constructed to the specification in the E&S plans and appeared to be functioning properly.

Six wells were observed to be in production phase during the inspection. Six GPU’s and two production tanks were observed on site. All conductivity readings taken fro

2620254 Primary Facility 2017-07-26 Routine/Complete Inspection No Violations Noted On July 26, 2017, the Department conducted a routine inspection at the Repsol Oil & Gas USA LLC (Repsol), Ugliuzza L 05-006 well pad in Pike Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Repsol.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The slopes of the pad were well vegetated. An area along the northern fill slope was observed to have an area of erosion. Sediment was observed to be deposited at the base of the topsoils stockpile. The Department recommends repairing the erosion gully. PCSM has been addressed by the installation of two bioretention basins. Several inches of water were observed in each basin due to recent heavy rainfall.

Six wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellars. Two production tanks and six GPUs were on site. Puddles on the pad and liquid with all containment systems were

2774977 Primary Facility 2018-09-11 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of the Repsol Ugliuzza (05-006) L Pad in Pike Twp, Bradford County on 9/11/2018 at 1230 hours. The inspection was completed in response to a Notice of Termination received by the DEP. The weather was 68 degrees, cloudy, and 86% humidity.

There are 6 producing wells onsite with two production tanks. All PCSM practices appear to be in place. The site appears to be well vegetated, stabilized and no temporary E&S BMPs were observed. A post drilling well site restoration report has been accepted by the DEP.

This Notice of Termination is recommended for approval. No violations noted at this time.
