2625496 Primary Facility 2017-08-01 Routine/Complete Inspection No Violations Noted On August 1, 2017, the Department conducted a routine inspection at the Repsol Oil & Gas USA Inc., Decristo Well Site in Warren Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Repsol.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was well vegetated and no erosion issues were observed. Two bioretention basins were observed to be installed at the site to address PCSM.

One well was in production during the inspection. The well was not labeled during the inspection. The Department recommends labeling the well as per 3211(h) of the Oil and Gas Act. No bubbling was observed in the well cellar. One production tank and one GPU were in secondary containment. There were no puddles or staining on the well pad surface. Liquid within the secondary containment structures was low in specific conductivity.

The well pad has not been reduced in size, but PCSM has been addressed at the

2799572 Primary Facility 2018-11-06 Site Restoration No Violations Noted NOT approval.

Electrical conductivity taken from pooled water on site with an Oakton PCSTestr 35. A puddle near the production tanks gave a 0.11 mS/cm reading. JLP
