0241090458 2019-04-02 Production Wells Violations Noted On this day, I am conducting an inspection per request of landowner Lawrence Woodruff in response to Complaint #6113. I found the well equipped with 4.5 inch diameter production casing, and 2 inch diameter tubing with the production valves closed. It is setup to be produced by means of a plunger lift system. There is a slight gas leak at the connection between the tubing and the rabbit catcher. The production facility consists of (1) 100 barrel steel tank, (1) 50 barrel steel tank, a drip tank, vertical separator, and an orifice sales meter with a current chart all contained within an earthen dike. There are old used absorbent pads present within the dike that will need removed. The tank is properly identified and the production records are current. It will be noted that there is an erosional concern along the lease road, creating a minor slip to occur. The road is currently passible, but will need repaired. Multiple pictures were taken. Additionally, there is oil staining at the end of the load line near the lease road entrance that will need addressed. The load line will also need buried in accordance with OAC 1501:9-10-05. An attempt to contact Andrew at AM-Tek Oil was made, and a voicemail was left stating that a compliance notice was being issued.

The well is currently shut in for repairs, but recent production is showing on the chart recorder. The well consists of 7 surface casing, 4.5 intermediate casing, and 2 production tubing with a rabbit catcher. There is legible and correct identification information at the tank battery, which is located approximately 100 from the well head. The tank battery consists of (1) 100 bbl, (1) 50 bbl tank, 1 mechanical separator, and a chart recorder.
