| 0807864037 | 2016-04-21 | Production Wells | Violations Noted | Performed an inspection of this well location with DOGRM Emergency Operations Coordinator Nathan Lane in response to a complaint of a brine release from the landowners property manager Marilyn McNabb. Upon inspection, it was observed that the well is in production and was in operation at the time of inspection. The well consists of a pumping unit standing over 4.5 production casing and 1.5 production tubing. The valves are in the open position on both strings. The tank battery consists of a 100 bbl production tank. The tank has leaked brine around a 1 valve at the bottom of the it. There is a brine scald area surrounding the tank and extending downhill of it encompassing an area 45 long by 20 wide. There is no containment dike around the tank. The tank is currently 27 away from the well, which violates DOGRM tank spacing requirements. Collins-McGregor Operating Co. will have 30 days upon the issue date of this notice to have the following completed: 1.) Pump down and move the 100 bbl production tank, excavate contaminated topsoil and dispose of it at an OEPA approved landfill; 2.) Replace contaminated soil with clean and apply lime/fertilizer and seed/mulch to the affected area to encourage regrowth of vegetation; 3.) Set a new production tank at least 50 from the well head and construct a containment dike around it to meet SPCC guidelines. |
