0900547998 2015-05-06 Production Wells No Violations On the day of my inspection I found a 8 X 4 wellhead with a 4 X 2 swage in the top. The valves were in the open position. The tank battery is made up of one 100 bbl tank, one 30 bbl brine plastic tank, a vertical separator, all within an earthen dike. The tank is properly identified and the production reporting is up to date. There are no violations at this time.
1100923005 2015-05-11 Production Wells No Violations On the date of inspection I am on site to determine the status of the well after the land owner, Donna Morris, called the New Philadelphia office concerning her domestic gas being shut off since Friday. I found production equipment to include a 100bbl tank, a 50bbl plastic brine tank, and a vertical separator within an earthen dike. The well is equipped with 8.625 inch casing, 4.5 inch casing, and a 4.5 to 2.375 inch reduction with a valve on top. One valve on the tubing is in open position and the other valve is in closed position leading to a domestic gas line. At the time of my inspection there was a swab rig at the well head. I left a message with Bruce Levengood with Sound Energy regarding the interruption of domestic gas service. ODNR Inspector Randy Akins was able to reach Bruce Levengood by cell phone and Mr. Levengood said he would take care of the domestic line as soon as possible.
