0772953355 2012-10-03 UrbanProduction Wells No Violations I completed an urban production well inspection status check. My inspection found the well equipped with a plunger lift unit surrounded by a chain link fence. The tank battery was surrounded by a chain link fence and contained one, 100 barrel oil storage tank, one, 50 barrel brine storage tank, a separator, and the proper identification. The production equipment had the proper spacing. I notice no visible spills or leaks. The lease road was in good condition. There are violations at this time.
0909814929 2011-03-19 Production Wells No Violations Waterwell Complaint , Paul Tanner [landowner] . Inspection showed annulus of production casing was closed . When company opened annulus it flowed approx. .75 bbl of fluid till there was 0 psi on annulus . There was approx. 40 psi on annulus . Well is located 2948 S-E of waterwell in question . I checked well on 3-21-2011 , there was still 0 psi on annulus .

1511684205 2015-02-23 UrbanProduction Wells No Violations I was on location for a status check. During my inspection, I found the well in production. The well was equipped with a plunger lift assembly. A domestic gas line came off of the 4.5 casing. The tank battery consisted of a 100 barrel production tank, a 50 barrel, poly brine tank and a mechanical separator. A dike surrounded the tank battery. An identification sign was properly posted. There was a plug on the load line. Locked, chain link fences with barb wire surrounded the well and tank battery. The lease road was in stable condition. The well was placed into production prior to House Bill 278.
