0002279039 2002-02-13 Preliminary Restoration No Violations 2-12-02 Received an anonomous complaint (Case # 02-003) about the drilling pits. 2-13-02 I inspected the pits and found the working pit very full of fluid and the pit liner on the south side of the blow pit had dropped into the pit. The drilling rig was stacked out on the location. 4-1/2 had been set on Sat. 2-9-02. Rig hands were working near the location. I told them to lift the liner or lower the fluid level in the blow pit. I later met with Steve Swain (Oil & Gas Solutions) and informed him of the complaint, my inspection and my recommendations. He said that they were going to pump out both pits and solidify them. NO VIOLATIONS.

1455770153 2018-04-23 UrbanProduction Wells No Violations Access to the tank battery was gained on a rock road. The access to the wellhead was in a hay field without any defined path. The well was operated as an electric pump jack. The wellhead was surrounded with a locked chain link fence and earthen dike. No leaks were associated with the well. I. D. signage was posted and legible at the well. The tank battery consisted of one (1) 210 barrel stock tank, one (1) 35 barrel poly brine tank and 2408 horizontal separator with meter. The Owner/ Operator was current in the reporting of the oil, gas and brine volumes. The load lines were plugged. I.D. signage was posted and legible. The tank battery was surrounded with a locked chain link fence and earthen dike. No violations were identified during this inspection.
