0093479832 2009-12-02 Production Wells No Violations The floor of the tank battery has been dusted with peatsorb in order to collect any additional crude oil which may surface. There was no free oil visible inside of the dike area.
0636979665 2008-07-16 Production Wells No Violations Met with Mr. Bartu on Farnham Road re: questions about his water well and the nearby spill. I explained the small amount of released product and the distance from the release point to his well made any correlation between his well odor and the spill very unlikely. I suggested that he chlorinate his well. If that doesnt work then he should contact a well driller to clean out his well.
0866359482 2009-11-12 Production Wells No Violations The steam cleaning inside of the containment dike was successful in recovering additional oil.
1215435460 2008-06-24 Production Wells No Violations Spill cleanup will continue with removal and disposal of impacted soil and vegetation. The tank battery will also be dismantled and cleaned.
1258669236 2009-11-10 Production Wells No Violations I received a call from Bill Kinney with Summit Petroleum reporting vandalism at this tank battery that resulted in a crude oil release into the containment area. The release was estimated at 80 bbl, none of which escaped containment. Summit initially responded with a vac truck and personnel. A steam truck, vac truck and personnel will respond later today to clean the impacted gravel and continue product recovery.
1334174035 2012-04-04 Urban Drill / Deepen / Reopen No Violations I received a call from Mr. Vinyard (440-376-0928) at 3040 Farnham Drive in Richfield. His concern was a sheen on one of the ponds on his property. Mr. Vinyards property is located approximately 800 west of the Nemetz tank battery. A small drainage ditch directs water from an area directly west of the Nemetz tank battery to the pond in question. The field inspection found no spills or other indications of a release at either the tank battery or the wellhead. The sheen on the pond appeared to be biologic in origin. The drainage ditch does originate in a residential area and proceed through a wooded lot where a ready supply of decaying leaves and vegetation are available. After inspecting the site, I spoke to Mr. Vinyard and gave him my thoughts on the origin of the sheen which appeared to satisfy his concerns.

1717752286 2008-06-23 Production Wells Violations Noted Bill Kinney with Summit Petroleum reported a dump line failure at the Nemetz tank battery. The leak occurred near the base of the separator and impacted the tank battery, ground surface to the west and a small nearby creek to the west. The loss estimate was 2 bbl of oil. The OEPA was notified of the spill since the creek was involved.
2098882788 2008-09-18 Production Wells No Violations The tank battery has been rebuilt and the impacted area has recovered. The violation has been corrected.
