1506002957 2017-10-06 Drill / Deepen / Reopen No Violations I did an inspection to check the status of the drilling rig. The rig was tripping the bit out of the hole and the hole started to unload. I spoke with Bobby Mitchell (Rig Owner) and he thought it may be unloading form the Newburg formation. The rig crew is trying to stop the hole from unloading with fluid which currently weighs 9.4 lbs/ per gallon. The weight goal for the kill fluid is 9.9 lbs/ per gallon. Pit liner is intact and the pit fluid levels are acceptable. No violations are noted at this time.
1506605532 2017-10-25 Urban Drill / Deepen / Reopen No Violations I did a status check for this well. The well is drilled but is not completed at this time. The rig has been loaded up for transport. I spoke with Dave Matak (J-well) and he advised me the pits should be solidified by 10-27-2017.
1507660185 2017-10-10 Drill / Deepen / Reopen No Violations I was on location to check the drilling status on Mitchell Drilling Rig #1. The rig was currently drilling at a depth of 4971 and will TD the hole in half a stand at 4993. The pits are in good condition and have plenty of freeboard. No violations.
1507820700 2017-10-11 Casing String Witnessed No Violations I was on location to witness the cementing of the 4.5 casing. Mitchell Drilling Rig #1drilled a 7.875 hole to a total depth of 4993. They began running 10.5lb/ft., 4.5 casing into the hole with a shoe on bottom, a ten foot shoe track, and latch down baffle. The casing was equipped with 12 bow spring centralizers. The bottom of the hole could not be logged due to a bridge at the bottom of the salt section at a depth of 4070. They ran 117 joints of casing to a depth of 4964 (KB +10) with a 12 landing joint. Petroset Cement rigged up and began circulating the hole. They pumped three (3) 10 barrel spacers separated by five (5) barrel freshwater spacers to condition the hole. They then mixed and pumped 43.4 barrels of SRRC (Rose Run) cement. They dropped the plug and displaced the plug to the baffle with 79 barrels of water. The plug landed and the pressure was increased to 1500psi and held for five (5) minutes with no leak-off. The pressure was released and the plug held in the baffle. No violations.
1508157930 2017-10-16 Drill / Deepen / Reopen No Violations I did a status check inspection on this well. The well is drilled and the production casing has been cemented. Diamond Oil Services is on location removing the pit fluids. The rig tower is down.
1521029276 2018-03-05 Preliminary Restoration No Violations I did a preliminary restoration inspection for this well. The preliminary restoration has not been completed. The ground is saturated and water is ponding throughout the location. I spoke with Dave Matak (J Well) and he advised me as soon as conditions permit the restoration will be completed by mid-May of 2018.
1652460267 2017-11-03 Production Wells No Violations I did a status check inspection for this well. The well has not been completed. The pits are closed. There some soil piles which need to be spread once ground conditions allow for further restoration. Currently the site is muddy.
1830056474 2017-10-10 Drill / Deepen / Reopen No Violations I received a call from Dave Maddick (Company Man) at 11:05 stating that he found dead fish in the unnamed stream 200 east of the drilling rig where freshwater was being pulled for cementing operations. I arrived on location at 11:21 and observed the sump area. There were several dead fish and many live fish in the deep area. I hiked 1000 upstream (north) and noted fish kill along the course. Dave walked the course almost to State Route 62 (upstream) and reported hundreds of dead fish in the stream and some up on the banks. I reported my findings to DOGRM Emergency Response Manager Terry Whitehair and he called dispatch at the Division of Wildlife to report the fish kill. There was no indication that the drilling activity or anything on location could have caused the fish kill upstream from the rig. Wildlife Officer Mark Basinger arrived on location to assess the issue. He said that the initial appearance was that it was some type of agricultural impact to the stream. He said he was going to get more officers involved to determine the source and the extent of the impact. I found no violations at this time.
1986427648 2018-10-15 UrbanProduction Wells No Violations Status check inspection of this Urban, Clinton Sandstone well. My inspection found the wellhead equipped with 2.375 rods and tubing and operating with the aid of an electric powered pumping unit. A gauge on the 4.5 production casing flow line indicates a pressure of 100psi and the ball valve is 1/4 open. There is a domestic tap on the casing port charged at 18psi and the supply line valve is currently closed. The belt guard is lying on the ground on the east side of the unit and it appears the motor has been replaced as there is another motor on the ground by the belt guard. I observed the rod clamp lightly tagging the top of the gland cap on the down stroke. I did not observe an ID sign at the wellhead. The tank battery is located to the northwest of the wellhead and is accessed behind 6020 Trojan Street. The battery is diked and contains two (2) 210 barrel tanks and a horizontal separator. There is no chart at the separator and the gas sales line valve is closed. Fluid is currently being produced into the tank battery. The tank hatch lids are closed and the vent stacks are equipped with charcoal filters and low pressure relief valves. There are two (2) identification signs lying on the ground within the tank battery and contain the required information. I will notify Inspector Murphy of my findings for follow-up and enforcement if necessary.
2052726776 2017-10-03 Drill / Deepen / Reopen No Violations I did a status check for this well. I checked in with Bobby Mitchell (Mitchell Drilling). The rig is drilling on air and the geolograph shows a depth of 2340 ft. There are no issues noted with the pits or the liner.
