0022077660 2014-08-14 Preliminary Restoration No Violations I was on location to witness the solidification of the pits for this well. At this time two cement trucks were on location pumping solidification material into the pits. A total of 4 loads have already been brought onto location and a 5th load may be needed. They expect to get the pits closed today so that Halliburton can come onto location tomorrow and log the hole to determine if the Rose Run will be productive. There are no violations at this time.
0275675747 2014-07-11 Administrative Inspection No Violations Ted Hill of Ohio Valley Energy has informed me that the Sidewinder Drilling rig will be moving on location Monday to begin drilling this well.
0790625033 2014-07-24 Drill / Deepen / Reopen No Violations I was on location to witness the remedial work done on this drilling location. The washout has been filled with stone and the need for 9sack grout or cement is not needed. Two steel road plates have been brought onto location and been put below the rig substructure to add to stabilization of the area. The rig was leveled and the Wildcat rig began rigging up and preparing continue drilling ahead. The rig picked the drill string off bottom and were able to circulate the well as there was concern the bottom hole assembly would have been compromised by setting it on bottom. At this time the rig is stable and will continue drilling the well at 2500. There are no violations at this time.
0862091756 2015-10-19 Preliminary Restoration No Violations I was on location to check the location for preliminary restoration of this well. The lease road is gated, stable and contains wellsite identification. The wellhead is fenced, contains identification and is currently a plunger lift assembly. The well contains all required pressure gauges and locks. The well is not currently producing and is awaiting a gas sales line. The tank battery is surrounded by a dike, fencing and is identified. The tank battery contains a horizontal separator, poly brine tank, 140bbl production tank and all required locks and plugs. The location has been seeded and vegetation has been established. Suitable vegetative screening has been planted and is currently growing. The tank battery, roadway and wellhead all conform to spacing requirements. Preliminary restoration has been passed at this time.
0983025678 2014-07-17 Casing String Witnessed No Violations

-cone bit. Following the reaming of the hole, a new guide shoe and new cement basket were installed on the casing before it was installed. A cement basket was installed on the casing at 81. The mine string casing was installed to 120. 300 pounds of bentonite gel were pumped ahead of 150sks of cement. The cement slurry was displaced with freshwater to cover the bottom of the void. An attempt was made to grout with 150sks of cement from the cement basket to the surface, but the 24 interval of sand encountered at the surface caved in, not allowing the cement to reach the basket. The grouting from surface sealed off the casing from surface and provided a water tight barrier for the well. 11.75 diameter surface casing will be installed next to a depth of approximately 350. The rig will wait eight hours to begin drilling out the cement. There are no violations at this time.

1073417464 2014-08-06 Drill / Deepen / Reopen No Violations The Wildcat rig is currently drilling ahead at 6550. The projected TD of the well is 6990. The rig is drilling on fluid at 45 per hour and expects to finish drilling the well sometime tomorrow morning. They will log the hole and determine if they are going to be producing the Rose Run Sandstone or plug the well back to the Clinton Sandstone. The rig has began slightly sinking again on location but it does not appear to be an issue at this time. They expect to be able to TD the well and install casing without any issues.
1093811806 2014-07-28 Drill / Deepen / Reopen No Violations I was on location to check the status of Wildcat Drilling. The rig is currently tripping back into the hole at 4619 measured depth and 4329 true vertical depth. The rig was showing sustained gas flow levels of 17 units on the gas meter and a visibly noticeable gas flow from the flow line. The gas could be smelled on location and registered on my PHD6 gas meter. I informed the rig that they would be no longer allowed to drill on air and that they must switch over to fluid drilling to kill the gas flow because this is an urban permitted well. The brine on location was not to the desired weight to load the hole with so salt had to be brought onto location to bring up the brine weight. During this time the pits were circulated through the blow out preventer and flow line to kill the gas flow. The brine was brought up to weight, the rig tripped back into the hole and began drilling on fluid as required. There are no violations at this time.
1291595778 2015-01-16 Preliminary Restoration No Violations I was on location to check the status of preliminary restoration. The sales line has been installed on location, the tank battery is partially installed and awaiting fencing. The wellhead is surrounded by a cage and has ID signage. When vegetation has been established and the tank battery completed I will follow up with an inspection.
1406309416 2014-07-23 Drill / Deepen / Reopen No Violations I was contacted by Ohio Valley Energy and informed that there has been a washout that has formed underneath the drilling rig. A large washout below the rig sub structure has formed and the rig floor is leaning and not fully supported. 16 tons of stone was brought in and put into the as support. 2500 of drill string was set on bottom so that the rig could be backed off of rig matting to evaluate the washout. The Wildcat rig was moved back from the well and the rig structure lifted up so that more rock could be filled into the washout. The leveling of the location and evaluation of the washout will continue tomorrow when equipment and fill material can be brought onto location.
1407444281 2014-08-07 Drill / Deepen / Reopen No Violations I was on location to check the status of Wildcat drilling this well. The rig just TD the well at 6988 measured depth. The rig is not going to log the well at this time, but is going to run 4.5 OD casing through the Clinton and Rose Run. They will cement the Rose Run and use a DV tool to cement the clinton as well. The rig is currently circulating the well and awaiting the production casing to be delivered. They expect at least 12 hours until cementing to begin. The rig has sunk slightly but has not affected the integrity of the well. There are no violations at this time.
1720652225 2014-08-02 Drill / Deepen / Reopen No Violations Received call from consultant for Ohio Valley stating they were re-installing their blow out preventer and needed an inspector to witness pressure test. Pressured up blow out preventer to 1200 PSI, held for 30 minutes with 0% decline in pressure. Test is passed.
