I found a tree root had punctured the liner at the top edge of the pit below the mud pump suction line. I talked to Charlie Keeney (Advent) and he informed me that they would reline the pit prior to drilling out from under surface. No violations.

Hugh Palmer had a crew on location running flow line from wellhead to the production unit. The location around the well was muddy and the topsoil was stockpiled to the north and will be spread and seeded when the location dries out. The working pit has been solidified and closed. Preliminary restoration is due by the end of March. No violations.

1235603319 2013-02-11 Completion Testing No Violations I was on location at 9:00am to witness the hydraulic fracturing of the Rose Run Sandstone. Universal Well Services was on location and rigging up to the wellhead. I met with Matt Egnotovich and Mike Weinsz (M&M Royalty) in the control trailer. 500 gallons of acid had been spotted in the hole after perforating last week. Universal held their safety meeting and tested the lines. The annulus on the surface casing was open before the start of the job. Universal began pumping, broke the formation down at 2070psi and shut down. The job went smoothly, but the well sanded off towards the end of the job. No pressure was seen on the annulus. See stimulation report for more details on this job. No violations.
1294454104 2012-12-27 Urban Drill / Deepen / Reopen No Violations I was on location to check the status of site construction. The location has been cut and graded. The topsoil and brush have been stockpiled on the north side of the location. Stone is being hauled in for the lease road. The proper I.D. has been posted on the lease road. No violations.
1359561875 2013-01-13 Urban Drill / Deepen / Reopen No Violations I was on location at 7:30 this morning to witness the cementing of the 4.5 production casing on this Rose Run well. The pits were in good condition and had plenty of freeboard. Advent Drilling drilled a 7.875 hole to a depth of 7112 and ran 7105 of new, 4.5 production casing with 10 centralizers. Casing was run with a float shoe on the bottom and a latch-down baffle at 7095. The float was not holding during the casing run. After the casing run, Advent began circulating the hole and Universal got spotted for the cement job. Universal rigged up, broke circulation, mixed and pumped 150 sacks of cement, released the plug, and displaced the cement and plug with 110 barrels of freshwater. The plug did not land and the well was shut-in at 925psi. I spoke with Matt (M&M) and he said they would wait 8 hours and then run in with a wireline to determine if the cement fell back. No violations.
1375884776 2013-06-11 UrbanPreliminary Restoration No Violations I was on location to check preliminary restoration status of this Rose Run well. The entire location has been graded and seeded and all debris has been removed. The wellhead is fenced with board on board fencing and the gate is locked. The access road is stoned all the way to the well head. The well is producing with a plunger lift system and shows 550psi on the wellhead. The proper ID is posted on the fence. The well is producing to a heated, horizontal separator and the setbacks are in excess of that required by OAC 1501: 9-9-05 from the wellhead and tank battery. The tank battery consists of 4-210 barrel tanks and one horizontal 200 barrel tank. This battery is surrounded by chain link fence and is sufficiently diked. The battery was constructed in 2011 for the Evans 1-K (#24413) and both wells are currently producing to this battery. The tanks are equipped with locked hatches, charcoal filters, and lightning arrestors. A small compressor has been installed at the north side of the diked area and is fenced and locked. Preliminary restoration is approved. No violations.
1419798585 2013-01-05 Urban Drill / Deepen / Reopen No Violations I was on location at 11:00am to witness the cementing of the 8.625 surface casing. Advent Drilling drilled an 11 hole to 789 and ran 760 of new surface casing into the hole. The hole was circulating good and no zones of lost circulation were reported. Universal Well Services rigged up, established circulation, and pumped 250 sacks of Type 1 cement. 15 barrels of good cement returned and held at the surface. See the cement/casing ticket for more details on this job. The pits were in good condition and had plenty of freeboard. Advent was going to re-line the working pit before drilling out from under the surface casing. No violations.
1768550631 2013-01-24 Urban Drill / Deepen / Reopen No Violations I was on location with Kevin Perry to check the status of this Rose Run well. Timco had their service rig sitting over the hole and were drilling out the cement and plug left in the production casing following the cement job. Timco tagged the plug around 6690 and were milling out the wiper plug. The blow pit had been solidified and closed and the working pit was in good condition and being used by Timco to circulate the hole. No violations.
1825659880 2013-01-04 Urban Drill / Deepen / Reopen No Violations I was on location at 1:00am to witness the cementing of the 11.75 conductor casing. Advent Drilling ran 1 joint of new conductor casing into the hole to a depth of 42 and began circulating the hole. Universal Well Services rigged up, broke circulation, and pumped 75 sacks of cement with good cement returns back to the surface. See cement/casing ticket for more details on this job. The pits were in good condition and had plenty of freeboard. No violations.
1913614424 2013-02-07 Urban Drill / Deepen / Reopen No Violations I was on location to review the bond log on this Rose Run well. M & M spent the past week drilling out cement left in the hole from the production casing cement job. I requested to be present in order to review the log and determine that the well was sufficiently cemented to continue with completion operations. Appalachian Well Surveys was on location and had run the bond log. They were currently running in the hole to perforate the well. I met with Matt Egnotovich to review the bond log. The bond looked good and there was good cement in excess of 500 above the top perforation. No remedial action will be required. No violations.
