0232315576 2013-07-01 Production Wells No Violations I was on location to inspect the progress of cleaning up the location following an oil release on 6/25/13. Sutton Pump and J. Hawk were on location and steam cleaning the inside of the tank battery dike and removing the free oil. The siphon dams were still in place and functioning properly. This clean up continues. No violations.

The location was beginning to dry out and there was currently no activity occurring on location. The siphon dams were in place and functioning properly. This clean up continues.
0591892511 2013-06-25 Production Wells No Violations I received a call from North Region Supervisor, Tom Hill, at 6:49pm about a lightning strike and oil release at this well location. I arrived on location at 7:40pm and the Palmyra Fire Department, Kurt Kollar (OEPA), Bob Allen (M&M), and Scott Miller Contracting were on location containing the spill. The lightning had struck a 210 barrel tank, blew the bottom of the tank out, and launched the tank to the west about 100 yards and came to rest in a wheat field. There was approximately 40 barrels of crude oil in the tank when the strike occurred. Most of the oil remained inside the dike and was being removed by Stallion. An unknown quantity escaped containment to the east and south on the location and some had migrated into a ditch running through the wheat field. I found no oil to the west near the tank and it was not causing contamination. Kurt Kollar had the cotractors install 4 siphon dams along the ditch to contain any oil running off location. The immediately, affected area was approximately 100x130 and I told Bob Allen that he could construct a lined pit, above grade, and place the affected top soil in the pit until it could be removed or remediated. They will also need to remove the equipment from the tank battery and steam everything inside the dike to collect the oil and steam or remove the affected gravel on the lease road to the point it is not causing any sheen. Kurt is overseeing the clean up of the ditch. This is not considered a negligent incident and no violation will be issued to M & M Royalty due to their immediate response and reporting of the spill. I will continue to monitor the progress of the clean up.
0723708399 2018-12-12 Production Wells No Violations Status check inspection of this Rose Run Sandstone well. My inspection found the well in operation and equipped with 2.375 tubing and rods and operating with the aid of a natural gas powered pumping unit. A gauge indicates a pressure of 300psi on the tubing. The required ID is posted on the Sampson post. This well produces to a diked tank battery to the north near the lease road entrance. The battery contains two (2) 210 barrel tanks, one (1) 100 barrel tank, and a horizontal separator. The chart at the separator indicates a differential production pressure between 50psi and 60psi. The required ID is posted at the tank battery. No violation.
1125279741 2013-06-27 Production Wells No Violations I was on location to follow up on a lightning strike and oil release that occurred at this location on 6/25/13. The damaged tank has been removed from the field to the west and is sitting on the side of the lease road. The affected topsoil has been scraped, mixed with lime and peat, placed in a lined pit, and covered for remediation. Heavy rains overnight have produced a sizeable amount of run off from the wheat field. I walked the ditch to check the condition of the siphon dams. The dams were functioning at near capacity and I called Bob Allen (M&M) to let him know that they needed to keep an eye on the dams after it rains so that they dont oveflow. No violations.
1220169716 2013-07-25 Production Wells No Violations I was on location to check the clean up status of the location following a lightning strike on 6/5/13. Scott Miller Contracting was on location and rebuilding the tank battery. 2-210 barrel and 1-100 barrel tanks were spotted in containment and being plumbed together. The remediating soil had been uncovered and was drying out. There was no noticeable free oil in the stockpile. The siphon dams were still in place and functioning properly. Several dozen minnows were present behind the siphon dam indicating good water quality. Several areas around the east side of the location still need to be addressed. This clean up is progressing.
1287654245 2013-07-08 Production Wells No Violations I was on location to follow up on an oil release from 6/25/13. Scott Miller Contracting was on location cleaning up the location. The tanks have been removed from the location and they will be bringing in new tanks this week. The inside of the dike has been cleaned and new gravel has been placed inside the dike. The siphon dams are in place and functioning properly. This clean up is progressing. No violations.
1551396877 2013-09-05 Production Wells No Violations I was on location to check the clean up status of this site following a lightning strike on 6/25/13. The tank battery has been rebuilt and the well is back in production. The remediated soil has been spread on location and shows no sign of free oil. There is a tarp and several garbage bags containing absorbent pads that need to be properly disposed. Site clean up is nearing completion. No violations.
