0832646712 2014-06-04 Orphan No Violations Plugging operations began on 6/4/14 and were conducted by Hagen Well Services, Appalachian Well Surveys, and Petroset Cementing. Inspector John Fleming witnessed the cementing of the bottom plug. Hagen set tubing at the bottom perforation and Petroset rigged up and broke circulation. They then mixed and pumped 6 barrels of cement slurry and displaced the slurry to 7,000 with 27 barrels of water. Hagen pulled tubing above the top of the Trenton and Appalachian ran a 10 shot perforating string into the tubing to perforate the 4.5 casing. The string sat down on paraffin at 5500+ and would not run any lower. The perforating string was pulled and 3 shots were missing from the string. Hagen Well Service ran back into the tubing with sinker bar and tried clear the tubing of paraffin. Appalachian tried to go back in but could not get through the paraffin. Rig shut down for the day. Waiting for new tubing.
1416334890 2014-06-10 Plug / Plug Back No Violations I was on location to inspect the plugging operations that concluded yesterday. Jeff Schonauer (JR Smail) was on location dismantling the tank battery. I gave him a copy of the landowner waiver to get signed. The plug has dropped and Jeff will top the hole off with Sackrete. No violations.
1426165209 2014-06-02 Plug / Plug Back No Violations I was on location to check progress towards plugging operations. Hagen Well Services was rigged up over the hole pulling rods out of the 2.375 tubing. Plugging operations will begin tomorrow. No violations.
1500651422 2017-07-21 Final Restoration No Violations I was on location to inspect the Final Restoration status of this plugged and abandoned Rose Run well that was plugged on 6/9/2014. My inspection today found that the area of the former wellhead is being cultivated with soy beans. The area of the former tank battery has been graded and grass is well established and part of the yard. All wellhead and production equipment has been removed. The old lease road is being used by the landowner for ingress and egress to the field. I have received the executed Landowner Waiver (Form 5) allowing for the flow/sales lines to remain buried in place. Final Restoration is approved.

They then mixed and pumped 6 barrels of cement slurry and displaced the slurry to 7,000 with 27 barrels of water. Hagen pulled tubing above the top of the Trenton and Appalachian ran a 10 shot perforating string into the tubing to perforate the 4.5 casing. The string sat down on paraffin at 5500+ and would not run any lower. The perforating string was pulled and 3 shots were missing from the string and operations were shut down for the day. I spoke with Steve Hagen the next morning and he informed that at least one shot went off inside the tubing and they could not get below 5500 and they lost 1300 of tubing down the 4.5 casing. A decision was made to set a 400 plug on top of the obstruction the next morning. On 6/6/14, 2.375 tubing was spotted at 5506 and Petroset pumped a 6 barrel cement plug and displaced the plug to 5106 with 20 barrels of water. Hagen pulled tubing above the Medina Sandstone and Appalachian ran in the tubing with a 10 shot string and sat down at 4310. The string was pulled and 9 of the shots had come off the perforating string. I had Petroset rig up to the tubing and circulate the tubing clean. The obstruction was cleared at 1200psi. Appalachian ran back in the tubing with 10-1 11/16 shots. They perforated the casing from 4684-4694, pulled the string, and all shots had fired. Tubing was spotted below the bottom perforation and Petroset pumped an 18 barrel plug displaced to 4394. Hagen pulled 35 joints of tubing, packed in the wellhead, and opened the valve on the surface casing. Petroset squeezed away 3 barrels of cement at 1000psi and the pressure was bled off. Hagen pulled the remaining tubing and plugging operations will resume on Monday (6/9/
2082842741 2014-06-09 Plug / Plug Back No Violations I was on location this morning to witness the final plugging of this well. Appalachian Well Surveys ran a bond log to find free pipe then ran in and shot the 4.5 casing off at 3450. Hagen pulled 12 joints to set the Big Lime plug and Petroset rigged up to the casing and established circulation. They then mixed and pumped 18 barrels of cement slurry and displaced the cement to 2638 with 42 barrels of water. I scaled the cement at 14.8lbs/gal. Hagen pulled 47 joints to set a plug at the bottom of the surface casing and Petroset rigged up and began circulating the hole. They then mixed and pumped 12 barrels of cement slurry and displaced the cement to 720 with 11 barrels of freshwater. Hagen pulled 17 joints to set a 200 plug at the top of the hole. Petroset rigged up, broke circulation, and then mixed and pumped 16 barrels of cement slurry. The cement circulated to surface and Hagen pulled the remaining 4 joints plus 20+ of the joint that was shot. The cement held at the surface and the casing will be cut off and fitted with an ID plate tomorrow. No violations.
