0201334083 2014-04-28 Plug / Plug Back No Violations My inspection finds the casing has been cut off at thirty inches below grade. There is no sign of any gas or oil migration through the plug.
0312774028 2013-09-12 Production Wells No Violations Called Mr. Reeves and discussed plugging the well. Due to the H2S I urged him to plug the well soon. He stated that he would plug the well as soon as the rig contractor he uses became free.
0926573686 2014-09-25 Final Restoration No Violations I am accompanied by Supervisor Rocky King. My inspection today finds the production equipment has not been removed. A 100 barrel tank, a 210 barrel tank, and wellhead equipment are present. I called Ted Cuttright, production superintendent, and he stated that they would remove equipment as soon as possible.
1106591486 2014-02-05 Administrative Inspection No Violations I called Mr. Jerry Reeves of David A Waldron & Associates at 11:11 am. At 11:23 am Mr. Reeves returned my call. Mr. Reeves stated that he has been in contact with a couple of service rig companies. He stated that plugging operations would commence within the next thirty days.
1251808826 2013-07-02 Production Wells No Violations Inspection revealed that well is equipped with 8 surface casing and 4.5 casing and 2.375 tubing and 1-100 bbl tank, company rep called and wanted me to come to site and test sulfur odor at wellhead it was determined that H2S was present at well and they placed ribbon around well and told landowner to stay away from well site until they can get plugging permit and rig lined up to plug said well.
1397673436 2014-04-16 Plug / Plug Back No Violations My inspection finds Berry Well Service has moved in with a service rig. The rig crew is trying to fish out any plungers or standing valves that may be in the tubing string. The bottom of the tubing was measured by wire line to a depth of 5821.
1397827021 2014-04-17 Plug / Plug Back No Violations On site to witness Berry Well Service try and work tubing free they were unable to do so and they attempted to pump on tubing and they were unable to do that also and decision was made to shot tubing off above seating collar in the morning and possibly set bottom hole plug.
1397942488 2014-04-18 Plug / Plug Back No Violations On site to set bottom hole plug they were unable to pump through tubing so decision was made to perf tubing above seating collar@5812 and they perfed@5797 and we pumped bottom hole plug top of plug was noted@5370 when we tagged it prior to getting telegraph on casing@4211 they pulled one joint of casing and will pull remaining casing on 4/21/14 and then run tubing back into well and set remaining plugs.
1398180631 2014-04-22 Plug / Plug Back No Violations My inspection today finds the service rig ran tubing to 2560 where it set down. Cemenco hooked up to the tubing and pumped until the tubing washed down through a small bridge. The rig crew then ran tubing to 4200. Cemenco spotted a 200 plug. The tubing was pulled up to 1782 at that point a 200 plug was circulated in place by Cemenco. A surface plug was circulated into place from 200 to surface. I instructed the company to remove all production equipment including a 4 pipeline that is above ground, and to reclaim the site in accordance with 1509:072.
