Regional Supervisor-Brent Bear, Muskingum County Sheriff-Matt Lutz and Deputy Sheriff-Pete Fisher. We met landowner Mr. Paul Dusenberry at the lease road entrance off Coopermill Road. The well is currently idle and not hooked up to a flowline, it is equipped with 4 1/2-inch diameter casing, 2 3/8-inch diameter tubing with rods sticking out of tubing, pumping unit has been removed. All valves are closed on tubing and casing, two heavy fence panels enclose the well. There is no identification posted at the well head. There has been no production reported to the Division of Oil and Gas since 1998 and all production tanks have been removed from the site. Mr. Paul Dusenberry stated he owns the well, he was informed he needs to provide the legal documentation showing transfer to him. The Division of Oil and Gas shows ownership belonging to Chesapeake Appalachia LLC. The Division conveys to Chesapeake Appalachia LLC to have the well plugged within two weeks after issuance of Compliance Notice or transfer the well to Mr. Paul Dusenberry with proper legal documentation prepared for the Division of Oil and Gas and sent to the attention of Bill Grubaugh. Identification will need to be installed at the well head within ten days of issue of Compliance Notice. The well owner/operator shall submit a written plugging schedule to Patrick Shreve at 5880 Memory Road, Zanesville, Ohio 43701 within 2 days per OAC 1501:9-11-05(B) and plug the well within 30 days. The well owner/operator will be notified if the plugging schedule is approved or denied. |
