0177175087 2018-05-17 Completion Testing No Violations Status check inspection found the well drilling process is complete and the well has the completion well head installed and has also had the 5.5 production string toe prep procedure and final pressure test conducted. The well is awaiting the start of the stimulation process in the next 10 days, Halliburton completion service will mobilize and begin. All completion equipment will be placed on polymer containment. All surface water created on the well pad due to the recent rain events is being removed by the means a vacuum truck. The perimeter drain valves are closed.

0295864327 2018-07-30 Completion Testing No Violations Status check inspection on the well completion process found Halliburton pressure pumping service on location along with Nine Energy wireline service. To date so far 57 out of a total of 67 proposed stages have been stimulated. Annular pressure monitoring is in place between the 5.5 and 9.625 casing strings with 67 pounds per square inch present.
0654302226 2018-04-05 Casing String Witnessed No Violations On this day, I am onsite to witness the 13.375-inch diameter surface casing cement job with Schlumberger. The H&P Flexrig 608 rig drilled a 17.5-inch diameter borehole to a depth of 1,772 feet from a KB of 27 feet. Then, Wyoming Casing inserted 408 feet of 13.375-inch diameter J-55 61.00 lb./ft. casing, and 1,314 feet of 13.375-inch diameter J-55 54.50 lb./ft. casing, for a total of 1,722 feet of casing into the wellbore. A pre-job safety meeting was held. Schlumberger initially pumped 260 barrels of fresh water, 20 barrels of gel spacer, followed by 10 barrels of fresh water. They cemented the casing with 299 barrels total volume of 1,422 sacks of Class A cement with a 1.18 yield, and a weight of 15.6 pounds per gallon, verified by scale. It was displaced with 258 barrels of fresh water. The plug landed with 730 psi and then pressured up to 1,380 psi. 55 barrels of cement returns were brought to surface.
1408204241 2018-08-16 Completion Testing No Violations Status check inspection found the well drilling and stimulation process complete. A snubbing unit type service rig owned by Deep Well Services is on location and is rigged up on the Yellow Rose A2H drilling out the completion plugs. The said well has had the completion plugs drilled out and is awaiting the initial production flow back operation stages. The well head is equipped with a lower A and upper B section with a production flow cross present above the B section followed by two manually operated FMC 7.0625 diameter master valves. No production tree or flow lines have been installed and none of the 4 wells on the pad have been or will be put into production while completion operations are taking place. Crews are on site installing the perimeter production equipment. All well operations are taking place on polymer containment liner and the well pad earthen berm is serving as the primary containment with the perimeter drain valves in the closed position while work is being done on the wells on the pad. Any surface water that is created through rain fall is being recovered and hauled off.

1525284176 2018-05-02 Drill / Deepen / Reopen No Violations Status check inspection found H&P Drilling rig 608 on location and currently rigging down preparing to move off the well pad. The rig drilled the said well to a measured depth of 23.819 , 5.5 diameter production casing was set to 23,772. All measurement were off a drilling rig Kelly Bushing heigth of 27.
1543420829 2018-11-28 Production Wells No Violations Status check inspection found 4 wells are present on the pad and all 4 wells are now in production. The said well is equipped with a lower A and upper B section with annular pressure monitoring in place on both well head sections. No annular pressure was found on this day on the A or B well head section. The well is producing off the 5.5 diameter production casing through (2) 3.5 diameter flow lines with both flow line valves open. The 5.5 casing pressure is 3,400 pounds per square inch (psi) and the flow line pressures are 1,400 psi each. There is one manual master 7.0625 completion valve still on the well head B section that the well is being produced through. Each of the (2) flow lines has a separate glycol heater and downstream from the Glycol heater is a contact tower and B-Tex Condenser followed by third party mid stream Eureka Hunter pipeline station point. Also on location is a production fluid facility that consists of (6) 400 barrel production tanks situated inside a polymer lined steel containment dike area. Proper well identification is posted and hazard warning labeling is present at the production facility. Signage is posted at the well pad entry point and the well pad perimeter is protected by an earthen berm.
