0191030206 2018-08-16 Completion Testing No Violations Status check inspection found the well drilling and stimulation process complete. A snubbing unit type service rig owned by Deep Well Services is on location and is rigged up on the Yellow Rose A2H drilling out the completion plugs. The said well has had the completion plugs drilled out and is awaiting the initial production flow back operation stages. The well head is equipped with a lower A and upper B section with a production flow cross present above the B section followed by two manually operated FMC 7.0625 diameter master valves. No production tree or flow lines have been installed and none of the 4 wells on the pad have been or will be put into production while completion operations are taking place. Crews are on site installing the perimeter production equipment. All well operations are taking place on polymer containment liner and the well pad earthen berm is serving as the primary containment with the perimeter drain valves in the closed position while work is being done on the wells on the pad. Any surface water that is created through rain fall is being recovered and hauled off.

1044051615 2018-02-05 Casing String Witnessed No Violations On this day, I accompanied ODNR Inspector Austin Gutridge to witness the cementing of the 13.375 inch diameter surface casing. Please reference his comments for a detailed report. Schlumberger pumped 1412 sacks of Class A cement with 1.19 yield, and 5.22 gal/sk. It was displaced with 262 barrels of fresh water and had returns of 50 barrels of cement to surface.
1049113402 2018-07-30 Completion Testing No Violations Status check inspection found Halliburton Pressure pumping service on location and stimulating stage 54 out of 58 total stages to be complete on the said well. Annular pressure monitoring is in place between the 5.5 and 9.625 casing strings with 173 psi currently present during stimulation operations. The initial break pressure was 10,300 pounds per square inch (psi) with an average treating pressure of 10,100 psi at a rate of 85 barrels a minute. The total stage fluid volume is 9,500 barrels of fresh water. Nine Energy wireline perforated the stage between the depths of 11,343 and 11,492 using 4 clusters of 6 shots per foot per cluster. All stimulation and support equipment is situated on polymer containment.
1458360758 2018-07-30 Completion Testing No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please reference his comments for a detailed report. We are onsite witnessing Halliburton, 9 Energy, and S3 Pump Service conducting hydraulic fracturing operations. This particular well is currently on stage 54, of 58 total stages. The plug is set at a measured depth of 11,522 feet. The bottom perforation is set at a depth of 11,492 feet, and the top perforation is set at a depth of 11,343 feet.
1518020862 2018-02-05 Casing String Witnessed No Violations The purpose of my inspection was to witness 1751 feet of 13.375 inch diameter surface casing being cemented into a 17.5 inch diameter open hole. The casing and casing equipment was evaluated on the pipe racks. HP 608 and Wyoming Casing crew ran the 13.375 inch diameter casing in a dry 17.5 inch diameter open hole. Schlumberger pressure tested their lines and pumped 280 barrels of fresh water, 20 barrels of 6% gel, 10 barrels of fresh water, and 300 barrels of class A cement weighing 15.6 pounds per gallon. A cement sampled was pulled and weighed in pressurized scales confirming the weight of 15.6 pounds per gallon. The top plug was dropped and 262 barrels of fresh water were pumped for displacement. Circulation was maintained throughout the job and when cement returned to surface a sample was taken and weighed in pressurized mud scales. The sample of the returns weighed 15.6 pounds per gallon. A calculated total of 50 barrels of cement returned to surface.
1547783100 2018-05-17 Completion Testing No Violations Status check inspection found the well drilling process is complete and the well has the completion well head installed and has also had the 5.5 production string toe prep procedure and final pressure test conducted. The well is awaiting the start of the stimulation process in the next 10 days, Halliburton completion service will mobilize and begin. All completion equipment will be placed on polymer containment. All surface water created on the well pad due to the recent rain events is being removed by the means a vacuum truck. The perimeter drain valves are closed.
1634334566 2018-11-28 Production Wells No Violations Status check inspection found 4 wells are present on the pad and all 4 wells are now in production. The said well is equipped with a lower A and upper B section with annular pressure monitoring in place on both well head sections. No annular pressure was found on this day on the A or B well head section. The well is producing off the 5.5 diameter production casing through (2) 3.5 diameter flow lines with both flow line valves open. The 5.5 casing pressure is 3,142 pounds per square inch (psi) and the flow line pressures are 1,400 and 1,600 psi. There is one manual master 7.0625 completion valve still on the well head B section that the well is being produced through. Each of the (2) flow lines has a separate glycol heater and downstream from the Glycol heater is a contact tower and B-Tex Condenser followed by third party mid stream Eureka Hunter pipeline station point. Also on location is a production fluid facility that consists of (6) 400 barrel production tanks situated inside a polymer lined steel containment dike area. Proper well identification is posted and hazard warning labeling is present at the production facility. Signage is posted at the well pad entry point and the well pad perimeter is protected by an earthen berm.
