0794000440 | 2017-10-19 | Drill / Deepen / Reopen | No Violations | On the day of my inspection, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. Upon inspection, H&P FlexRig 608 is tripping back in the hole and going back to bottom, after successfully recovering a fish. The most current survey, at a depth of 18,391 feet, had an azimuth of 314.84 degrees and an inclination of 91.35 degrees. Projected total depth is 26,089 feet. |
0892998317 | 2018-07-19 | Production Wells | No Violations | Status check inspect as a follow up to complaint # 5883, the complaint nature is a odor in the area of the residence. The well was observed to be equipped with a 13.375 diameter casing string along with a 9.625 diameter and 5.5 diameter casing string. The well head is equipped with a lower A section and upper B section and producing off the 5.5 production casing through a primary master valve and production tree with multiple secondary master valves in place along with a emergency shut down valve (ESD) that is remotely controlled both on site through emergency shut down buttons and off site by electronical means. The A section which is the 9.625 x 13.375 annular has a pressure monitoring gauge in place along with the B section which is the 5.5 x 9.625 annular space which also has a pressure monitoring gauge in place with both annulis reading zero pressure. The well is flowing into a Glycol dehydration unit followed by a contact tower and a line heater. There is a production fluid facility present consisting of 5 steel construction 400 barrel capacity production tanks situated inside a polymer lined steel containment dike area, besides the production facility is a low pressure recovery combustion unit. There also is a Eureka mid stream gas distribution point on the well pad. I informed the well tender on site that I was responding to an odor complaint and asked them to monitor their pressures and address any line purging activity. |
0981109327 | 2018-08-15 | Production Wells | No Violations | Status check inspection to document the well is in production and there is also a rig identified as H&P 608 on location and currently drilling on 3411124838-0000. The company has submitted a simultaneous operation plan and is adhering to the plan language. The well is flowing through (2) 3 diameter flow lines into (2) gas processing units. The rig is being powered off the gas produced from this well. |
1504745459 | 2017-09-06 | Not Drilled | No Violations | On this day I met on site with Bruce Carpenter with Eclipse Resources and members of the US Beau of Land Management (BLM). The well pad construction certification has been approved by the Divisions Engineering staff and the well drilling permits have been issued. The Company wishes to spud the wells however the (BLM) has indicated due to (2) of the wells on the pad are being drilled into a Federal Mineral a Federal Drilling permit must be issued and other Federal Permit Requirements regarding the well pad construction must be met before the wells can be spudded. Also noted is the well pad location is on private and not Federal property. |

1526581432 | 2018-05-17 | Completion Testing | No Violations | Status check inspection on the well completion process found Halliburton Pressure pumping service on location along with Nine Energy Wireline service. Halliburton is currently stimulating stage 82 out of 96 stages to be completed. The stage was perforated by Nine Energy with 36 perforations diveded up in 4 clusters, a solid bridge plug was set at a depth of 12,967. The well bore annular pressure on the 9.625 and 13.375 casing sections are being monitored daily by the means of mechanical gauges and the company man on site is reporting no annular pressure. |

1622052059 | 2017-10-03 | Drill / Deepen / Reopen | No Violations | On the day of my inspection, I accompanied ODNR Engineer Technician Andrew Thomas. Upon inspection we found the H&P FlexRig 608 is currently drilling the 12 1/4 inch diameter borehole on air, at a measured depth of 8,330 feet. We walked around location and found standing fresh water within containment at the base of the 500 barrel steel tanks. The company man was informed and the fluid will be cleaned up. |
1739592987 | 2018-06-07 | Surface Facility Construction | No Violations | On this day, I am conducting an inspection by request of ODNR Inspector William Stolarik in reference to an air quality complaint by a landowner. No unusual smells are indicated at this location. The pad is in the process of being prepped for production. Containment facility is being installed, as well as production flowlines. |
