0241322560 2017-10-03 Completion Testing No Violations On the day of my inspection, I accompanied ODNR Engineer Technician Andrew Thomas. We were on location to examine a fluid release that occurred. Upon inspection, we found Halliburton on location completing the wells and S3 Pump Service running logging tools down hole. Also, we observed cleanup efforts for recent release. It was indicated that flange bolts broke on the high-pressure side of a pumping unit and released flowback fluid on containment and the pad surface. It was stated that the release contained approximately 40 barrels of flowback fluid. Contaminated materials are being dug up and stockpiled, then hauled from location in roll-off bins by Cowboy Industries. The materials will be taken to a facility to be tested. Containment liner beneath the frac trucks was ripped and indicated seepage to the pad surface. Small trenches are being dug to assist in the collection and removal of fluid. More environmental investigation may be needed to assess total impact to the pad surface and sub-surface. Bruce Carpenter was contacted by Andrew Thomas and explained the situation. Bruce said he would contact ODNR when they would do the investigation.
1088738644 2017-02-25 Casing String Witnessed No Violations I was on location to witness Stingray Cementing cement the 24 inch diameter casing into a 30 inch diameter bore hole. Pioneer drilled a 120 foot 30 inch diameter bore hole and set 120 feet of 24 inch diameter casing into the hole. Stingray Cementing ran 15 barrels of fresh water gel ahead of the cement. Stingray Cementing then ran 290 sacks of class A cement, 2% CaCL, and .25 parts per sack of floseal with 62 barrels of fresh water at 15.6 pounds and a yield of 1.2. Stingray Cementing then displaced with 55 barrels of fresh water until we got cement to surface. Stingray Cementing displaced about 15 barrels of cement to surface and finished cementing the casing into the bore hole.
1496567076 2017-06-04 Casing String Witnessed No Violations On site inspection at 3:30 am on 06/04/2017 to witness the surface casing cement job with H&P Drilling rig 608 on location along with Schlumberger Cement Service. Upon arrival it was observed that the rig has drilled a 17.5 diameter surface borehole to a measured depth of 1,950 measured off of a drilling rig Kelly bushing of 27. The rig then ran a total of 1,859 of 13.375 diameter J-55 grade buttress thread casing with a casing guide shoe ran on bottom and a float collar ran at the 1st collar off bottom completing a 46.24 shoe track assembly. Proper casing centralization was demonstrated with a casing centralizer placed every 4th casing collar. The cement company assisted in circulating the last casing joint into the well bore with circulation established at that point prior to cementing. Upon the landing of the last casing joint the cement head was installed with the wiper plug present and the cement job pumped with a 20 barrel 6% gel spacer pumped ahead of the 320 barrel cement mix consisting of 1485 sacks of Class A cement weighing 15.6 pound per gallon with the cement weight verified by weight of scale. The cement was displaced to the float collar with 280 barrels of fresh water with cement returns witnessed to surface at 235 barrels into displacement and the casing pressure tested to 1,325 psi upon the landing of the cement plug at 9:34 am. The final displacement pressure was 804 psi.
