0429488855 2017-10-03 Completion Testing No Violations On the day of my inspection, I accompanied ODNR Engineer Technician Andrew Thomas. We were on location to examine a fluid release that occurred. Upon inspection, we found Halliburton on location completing the wells and S3 Pump Service running logging tools down hole. Also, we observed cleanup efforts for recent release. It was indicated that flange bolts broke on the high-pressure side of a pumping unit and released flowback fluid on containment and the pad surface. It was stated that the release contained approximately 40 barrels of flowback fluid. Contaminated materials are being dug up and stockpiled, then hauled from location in roll-off bins by Cowboy Industries. The materials will be taken to a facility to be tested. Containment liner beneath the frac trucks was ripped and indicated seepage to the pad surface. Small trenches are being dug to assist in the collection and removal of fluid. More environmental investigation may be needed to assess total impact to the pad surface and sub-surface. Bruce Carpenter was contacted by Andrew Thomas and explained the situation. Bruce said he would contact ODNR when they would do the investigation.
0731984736 2017-02-19 Casing String Witnessed No Violations I was on location to witness Stingray Cementing cement the 24 inch diameter casing into a 30 inch diameter bore hole. Pioneer drilled a 120 foot 30 inch diameter bore hole and set 120 feet of 24 inch diameter casing into the hole. Stingray Cementing ran 15 barrels of fresh water gel ahead of the cement. Stingray Cementing then ran 290 sacks of class A cement, 2% CaCL, and .25 parts per sack of floseal with 62 barrels of fresh water at 15.6 pounds and a yield of 1.2. Stingray Cementing then displaced with 57.7 barrels of fresh water until we got cement to surface. Stingray Cementing circulated about 15 barrels of cement to surface and finished cementing the casing into the bore hole.
1493234952 2017-04-26 Casing String Witnessed No Violations On site inspection accompanied by Inspector Brian Sefcik to witness the surface casing cement job. H&P rig 608 is on location and has drilled a 17.5 surface casing cement job to a measured depth of 324 and ran 304.8 of 13.357 diameter J-55 grade buttress thread casing with a float shoe ran on the bottom of the casing and a float collar an at 259 measured depth from surface, 3 casing centralizers were also ran. Schlumberger Cement Service cemented the casing using a 72 barrel class A cement mix consisting of 340 sacks of cement. Schlumberger pumped a 20 barrel 6% gel spacer ahead of the cement. Upon releasing the cement plug the cement was displaced with 40 barrels of fresh water at 5 barrel a minute and 110 psi with the casing was pressure tested to 1004 psi upon the landing of the cement plug. No cement returns were witnessed to surface and the company was notified prior to cementing that the cement volume they proposed for the job only included 4 sacks excess cement over perfect hole volume of 336 sacks. The company ran 1 tubing to 100 and tagged cement, then then proceeded to top off the annular space with a 18 barrel cement mix with cement returns reported to surface.
1534869196 2018-08-21 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072 with erosion control and vegetation established. The lease road and well pad is of stone construction and in good repair. The well pad perimeter is protected by an 8 chain link fence with signage posted at the entrance point. There are 5 wells present on the pad 3 Utica and 2 Marcellus. Each well is properly identified with signage. Fire extinguishers and emergency shut down devices are present at all critical location points on the well pad. Addressing the well - The well is equipped with a lower A and upper B section with a production flow cross incorporated into the B section on the Marcellus wells. The well is equipped with a production tree consisting of a manually operated primary master valve and a emergency shut down valve. Present on the well head A & B sections are 2 diameter riser pipes equipped with valves and gauges to monitor annular pressure. The A section gauge that measures the pressure between the 9.625 and 13.375 diameter casing strings shows zero pressure and the B section gauge which measures the pressure between the 5.5 and 9.625 diameter casing strings measures 400 pounds per square inch (psi) pressure. The 5.5 diameter production casing internal working pressure is 1,200 psi and the production flow line is 500 psi. Production equipment on site includes multiple line heaters a contact tower and glycol heater along with a gas compressor. The production fluid storage area consists of (10) steel construction 400 barrel fluid capacity tanks situated inside a polymer lined steel containment dike area with well and hazard identification present along with fire extinguishers and a emergency shut down control point. The company has also added an additional production fluid storage area consisting of (5) steel construction 500 barrel fluid capacity tanks.
