0069880047 2016-12-21 Completion Testing No Violations On the day of my inspection I witnessed the well was flowing back with through Tetra Services flowback equipment to sales with 6200 pounds per square inches on the casing at 7 MMCF per day and 54 barrels per hour through a 24/64 inch choke. I met with Consultant Cristo Giannareas and completed walk around.
0545097854 2017-01-05 Production Wells No Violations On the day of inspection accompanied by ODNR Senior Health Physicist Paul Carder, I witnessed the well was online with the valves in the open position. Gauges present read 7150 pounds per square inches on the casing and 6150 pounds per square inches on the tubing with annular monitoring in place. Production equipment included a sand trap, a line heater and a 2- stage vertical separator. Also present included 4-400 barrel tanks within containment, 2 filter separators, 2 vertical scrubbers, 2 heater treaters and a gas combustor all shared with 34111246100000 Pittman 3H, 34111245920000 Holliday A 1H, 34111245910000 Holliday A 3H and 34111245960000 Holliday B 7H. We met with Eclipse EHS Technician Ken Durst and completed walk around revealing containment was being removed. I instructed Mr. Durst to fill the mouse holes in accordance with OAC 1501:9-11-11 and to continue to pull fluids out of the sump until the levels are acceptable.

-110 Casing was ran into the well bore with a float shoe and collar ran along with (3) Nine Energy -Smart Start flow guns in the casing string at 21,189-21,139&21,092. Schlumberger is on location to cement the casing using a 60 barrel Mud Push 15.lb.gl pre cement sweep followed by 4410 sacks of Class A-15.6 lb/gl 1.19 yield Cement displaced with 452 barrels of H2O with the final circulating pressure of 4,180 psi and the plug bumped at 4,940 psi, 800 psi over. The job design was for a calculated top of cement on the 5.5 annular of 431

1029667525 2018-05-22 Production Wells No Violations Status check inspection found the well drilling and completion process now complete and the well producing through 2.875 production tubing set at 10,183 with 2 manual and 2 emergency shut down in line valves present in the production flowline. Deep well service tubing snubbing unit is on site and finishing up switching out the production tubing 2.875 string. FMC well head is on site and has reassembled the well head consist and pressure tested the production tree stack. Proper well identification is posted. Annular pressure monitoring is in place on the 13.375 and 9.625 well head sections with zero pressure on the 9.625 and 200 psi on the 13.375 well head sections. Production related equipment on site consists of 5 gas processing in line heaters and 5 sand trap separators along with a single glycol heater. The production fluid facility consists of 4 steel construction tanks of 400 barrel capacity situated inside a polymer lined steel containment area.
1440015582 2015-08-19 Casing String Witnessed No Violations The said well was spudded on 08/17/2015 by Precision Rat Hole with a 30 x 120 conductor borehole being drilled and 120 of 24 .375 wall 94.62# pound per foot weld connection casing being ran. Universal Well Service is the cement contractor on location to cement the casing using 320 sacks of class A-3% calcium .25 pound per sack loss circulation material with the pipe capacity being displaced with 53 barrels of fresh water and 10 barrels of 6% gel prior to cementing with good cement witnessed to surface. Inspector Brian Sefcik accompanied me on this day.
1466199875 2016-06-17 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job found H&P Drilling rig # 531 on location with the 17.5 production borehole drilled to a measured depth of 1,760 with a total of 1723 of 13.375J-55 Buttress thread casing ran along with the following casing equipment -a float shoe and float collar with proper centralizer placement demonstrated. Circulation was established during the running of the casing and maintained throughout the cement job with 20 barrels of 6% gel spacer ran ahead of the 1,334 sacks of class A cement. The casing was pressure tested to 500 psi over displacement pressure upon the landing of the cement plug with the floats holding and good cement returns to surface, the casing pressure test showed a initial pressure of 1,300 psi with a 75 psi loss in the 1st 5 minutes, it was observed that the cement head was leaking and the company was directed to test the casing when the BOP stack was installed. A casing pressure test was submitted with the BOP test the following day.
1475167062 2016-09-29 Completion Testing No Violations Status check inspection on the well completion process found Halliburton pressure pumping service on location stimulating stage 29 of 64 total stages to be stimulated. Nine Energy wireline services perforated the stage from 16,895 to 17,001 with 36 perforations in 3 clusters with a completion plug being set at 17,304. FMC Well Head is providing the completion well head. The annular pressure is being monitored electronically.
1594290148 2016-06-22 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling process found H&P Drilling rig 531 on location with the 12.250 1st intermediate well bore total drilling depth recently reached at a measured depth of 9,484. The company is currently running the 9.625 casing at a measured depth of 8,088and has stopped running casing to load the casing and hole with processed water just below the Clinton formation.

1820372964 2017-03-21 Preliminary Restoration No Violations Preliminary Restoration found the area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The lease road is in good repair and stoned. The location entry point is posted with the owners contact information. The location is fenced with chain link fence. There is a total of 6 wells on the pad 5 of which are complete and all 6 have identification posted. The said well has a casing and tubing well head in place and is equipped with tubing head valves and B section cross valves. There is a Hydraulic operated electrically controlled emergency valve on the tubing head output flow line that has shut down points at the well head and production facility along with at the well pad entry point. Also on location downstream from the well head is a sand trap and line heater and then a mechanical separator. The mechanical separator sends the processed fluid to the production facility that consists of 4 steel construction 400 barrel capacity production tanks located inside a polymer lined steel containment area. There is also 2 dehydration units on location that service the 6 total line heaters on the pad.
