0622911269 2017-01-05 Production Wells No Violations On the day of inspection accompanied by ODNR Senior Health Physicist Paul Carder, I witnessed the well was online with the valves in the open position. Gauges present read 7165 pounds per square inches on the casing and 6350 pounds per square inches on the tubing with annular monitoring in place. Production equipment included a sand trap, a line heater and a 2- stage vertical separator. Also present included 4-400 barrel tanks within containment, 2 filter separators, 2 vertical scrubbers, 2 heater treaters and a gas combustor all shared with 34111246100000 Pittman 3H, 34111245920000 Holliday A 1H, 34111245910000 Holliday A 3H and 34111246090000 Circosta 1H. We met with Eclipse EHS Technician Ken Durst and completed walk around revealing containment was being removed. I instructed Mr. Durst to fill the mouse holes in accordance with OAC 1501:9-11-11 and to continue to pull fluids out of the sump until the levels are acceptable.
0728258355 2016-12-13 Completion Testing No Violations On the day of my inspection I witnessed Deep Well Service Stand Alone Snubbing Unit # 9 (DWS # 9) had drilled out the last frac plug # 7at 120602 feet with a 4.5 inch diameter tricone bit and was tripping out of the wellbore. DWS # 9 was preparing to run 2.875 inch diameter 6.7 pound per foot production tubing which will be set at a projected depth of 10583 feet. Tetra Services was onsite controlling well pressure at 4300 pounds square inch along with flowback of fluids.
0900055829 2017-03-21 Preliminary Restoration No Violations Preliminary Restoration found the area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The lease road is in good repair and stoned. The location entry point is posted with the owners contact information. The location is fenced with chain link fence. There is a total of 6 wells on the pad 5 of which are complete and all 6 have identification posted. The said well has a casing and tubing well head in place and is equipped with tubing head valves and B section cross valves. There is a Hydraulic operated electrically controlled emergency valve on the tubing head output flow line that has shut down points at the well head and production facility along with at the well pad entry point. Also on location downstream from the well head is a sand trap and line heater and then a mechanical separator. The mechanical separator sends the processed fluid to the production facility that consists of 4 steel construction 400 barrel capacity production tanks located inside a polymer lined steel containment area. There is also 2 dehydration units on location that service the 6 total line heaters on the pad.
1105374603 2016-08-16 Drill / Deepen / Reopen No Violations On site inspection follow up to violation # 1470707550 found the company has removed the drilling rig. The containment is still in place on the well pad and no remedial work or soil testing has been done. During the previous days prior to 08/16/2016 multiple rain events did occur in the area and the well site drain valve located at EPE 34 -39.77652 X 80.92439 was found open and the company was discharging fluid from the well pad generated from the rain event. Chloride levels were tested and found to be 475 parts per million at the exterior well pad drain discharge point.
1468063727 2016-07-09 Casing String Witnessed No Violations On site inspection to witness the 13.375 surface casing cement job found H&P drilling rig 531 on location with a 17.5 surface borehole drilled to a depth of 1,765, the company ran 1,694 of 13.375 54.5lb/ft J- 55 casing with guide shoe and float collar ran. Schlumberger Cement service cemented the casing using 1390 sacks of class A cement with 20 barrels of 6% gel spacer ran ahead of the cement.

1470707550 2016-08-08 Drill / Deepen / Reopen Violations Noted On site status check inspection found H&P Drilling rig 531 on location with the well bore drilling process recently completed and the 8.5 total measured depth drilled to 20,884. The rig is breaking down and moving off location. Also noted on a area of the well pad located at EPE of 9 39.77650 x 80.92514, a white substance was observed on the ground surface and was traced back to the primary containment. In the area of the substance, 2 different pools of standing fluid was titrate tested for chlorides and each area exceeded a low level Quantab strip test of 644 parts per million and measured 1,722 parts per million on the high level Quantab test strip, indicating the presence of brine water on the well pad surface off of the primary containment. Photographs were taken and my direct supervisor Brent Bear along with ODNR Hydrologist Jake Glascock was informed of my findings. The company men on site were made aware of the situation as well as Bruce Carpenter with Eclipse Resources with the following directions given moving forward; Recover all standing fluid off the well pad as well as any affected surface material.

1672279966 2018-05-22 Production Wells No Violations Status check inspection found the well drilling and completion process now complete and the well producing through 2.875 production tubing with 2 manual and 2 emergency shut down in line valves present in the production flowline. Proper well identification is posted. Annular pressure monitoring is in place on the 13.375 and 9.625 well head sections with 2,300 psi on the 9.625 and 0 psi on the 13.375 well head sections. Production related equipment on site consists of 5 gas processing in line heaters and 5 sand trap separators along with a single glycol heater. The production fluid facility consists of 4 steel construction tanks of 400 barrel capacity situated inside a polymer lined steel containment area. 6 wells total are present on the pad.

1984590846 2016-12-21 Completion Testing No Violations On the day of my inspection I witnessed the well was flowing back with through Tetra Services flowback equipment to sales with 6400 pounds per square inches on the casing at 17.8 MMCF per day and 22 barrels per hour through a 23/64 inch choke. I met with Consultant Cristo Giannareas and completed walk around.
2006236354 2015-08-16 Casing String Witnessed No Violations The said well was spudded on 08/14/2015 by Precision Rat Hole with a 30 x 120 conductor borehole being drilled and 120 of 24 .375 wall 94.62# pound per foot weld connection casing being ran. Universal Well Service is the cement contractor on location to cement the casing using 320 sacks of class A-3% calcium .25 pound per sack loss circulation material with the pipe capacity being displaced with 53 barrels of fresh water and 10 barrels of 6% gel prior to cementing and good cement returns witnessed to surface.
