1098938473 2018-02-27 Production Wells No Violations Status check inspection found the well pad lease road has signage posted and is gated and locked. The well pad has an 8 chain link fence that is also gated and locked with signage posted at the entrance also. There is also a emergency shut down device at the well pad entrance. On the well pad there is 2 active wells present with annular pressure monitoring in place on each well and the well tender reporting no abnormal pressures recorded or encountered. Each well head is configured with a casing flow cross and (2) outlet valves in place however no casing flow lines are attached. Attached to the tubing heads are (2) manual and (2) remotely controlled emergency shut down valves. Each of the 2wells on location has proper well identification posted. Also on location is a large natural gas compressor followed by a gas processing unit and 5 gas dehydration units, one supporting each well on the pad. At the far left hand side of the well pad is the production fluid storage area consisting of a total of (4) tanks that are steel construction and situated inside a polymer lined containment area.

1459873437 2016-04-05 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well site construction meets ORC 1509.072 with erosion control and vegetation established, the well pad perimeter is enclosed in a with a 8 fence and locked gate. Addressing the well head - on this day it was observed that the well is equipped with a 2.875tubing head with the production valves in the open position, equipment also present on the well head is (2) manual shut down valve and (2) General Electric Hydraulic emergency shut down actuators with a primary well pad shut down button located entry point of the location outside the well pad perimeter gate. Also on location is 2 Dehydration gas processing units and 2 mechanical fluid separator units with 3 -400 barrel production tanks present inside of a polymer lined metal containment dike area.
1956908982 2015-07-08 Casing String Witnessed No Violations I am onsite to witness the conductor cementing operations. Precision Rathole drilled 113 of 36 borehole and installed 113 of 30 weld casing. Universal cemented the casing in place with 109 barrels of Type 1 cement with 3% CaCl and .25 pounds per sack of Flowseal. I witnessed 35 barrels of returns to surface. Eighteen inches of cement was left in the cellar to provide a liquid tight seal.
