1425678415 2015-03-06 Casing String Witnessed No Violations On site inspection to witness the setting of the 18.625 mine string to a depth of 357 inside a 21.630 diameter borehole drilled to a depth of 359 by H&P drilling rig 531. A coal seam was encountered @ 311 to 314 with no circulation loss encountered while drilling or cementing. Schlumberger Cement service was on location to cement the casing with 340 sacks of class A cement with circulation established prior to cementing and 20 bbl of 6% bentonite gel ran ahead of the cement. The cement was displaced to 337 with 104 barrels of fresh water and the casing shall be pressure tested by the rig prior to moving foreword with the drilling process. Good cement returns were witnessed to the cellar.
1425839268 2015-03-08 Casing String Witnessed No Violations H&P Rig 531 has now drilled a 17.5 x 630 surface casing borehole and set 1,602 of 13.375 61# J-55 casing. Schlumberger cement service was on location to cement the casing using 1210 sacks of class A cement with circulation established prior to cementing and the casing pressure tested to 500 psi over lift pressure upon the landing of the cement plug with good cement returns witnessed to surface.
1426809434 2015-03-19 Drill / Deepen / Reopen No Violations Administrative inspection to document the reported recent well bore gas influx of 200 psi on the annular casing and 2800 units of 3rd party measured gas. The current oil based mud weight is 15.5# going in and 15.6# going out of the well bore. The total measured depth is 15,230 and TD will be 16,768 in the directional azimuth of 148.4 degrees. The company will complete a circulation cycle and the begin to bring their mud weight up before proceeding on.
1427206765 2015-03-24 Casing String Witnessed No Violations 03/24/2015 Inspector on site to witness the 5.5 production casing long string cement job. The well has been drilled to a measured depth of 16,769 in the directional azimuth of 248.17 degrees with a true vertical depth of 10,177 at the landing point. 16,766 of 23# 5.5 casing has been set in the well bore with 4 flow gun type perforating system included into the casing string from 16,628 to 16,466. Upon my arrival @ 8:15am to witness the cement job I was notified that the Schlumberger cement pump truck has experienced an equipment issue and it would be at least 4 hours before the job would start.

1465227237 2016-06-06 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the area affected by the well pad construction meets ORC 1509.072 with erosion control and vegetation established . 3-400 barrel capacity storage tanks are on site located in a polymer lined steel containment dike.
1520519101 2018-03-08 Production Wells No Violations On site inspection to document the well head annular pressures. The 13.375 well head had (0) zero pressure and the 9.625 well head had 800 (PSI) pounds per square inch pressure. The production tubing valves were open along with the Emergency Shut Down valve. The location has several emergency shut down control points identified at the well pad entrance- at each of the (3) wells total on the well pad - at the production fluid facility and the gas processing units. The well is producing of the tubing head assembly. On this day I spoke with a company HSE representative identified as Bruce Carpenter and we discussed the annular pressure on this well and the rest of the wells on the pad. I have asked the company to monitor and record all annular pressures and address any problematic wells that may have pressures present that could compromise the well head or casing integrity.

1658910774 2014-12-30 Casing String Witnessed No Violations The said well was spudded on 12/28/2014 by Precision Rat hole Services with a 120 x 30 conductor borehole being drilled and 120 x 24 conductor pipe ran and set. Universal well service is on location to cement the conductor using 305 sacks of type 1 2% calcium .25 pps flake 40% open hole excess cement. The mix water was tested prior to cementing with the chlorides levels reading 56 parts per million and PH level reading 7.
