On this day it was observed that the production tubing head had a pressure of 1,088 psi with the 5.5 showing 1,362 psi at surface internally- the annular of the 5.5 showed 530psi and the annular of the 9.625 showed 907 psi. The production tubing sales line showed 1,066 psi.

The drilling company Alpha Hunter rig 9 is currently reaming the borehole with a 17.5 tri cone drill bit at a depth of 2309. Once proper hole conditions are met the company shall run and cement casing.

A 22 borehole was drilled to 815 and 813 of 18.625 was ran and set. The mine void was encountered at 756 with historic records showing a 6 coal vein, a balance cement job was done on the bottom section and the cement was tagged at 767 inside the casing, The company then waited 8 hours and started cementing above the basket set at 728 through 1 tubing ran in the annular space to 475, the company is waiting 6 to 8 hours between stages.

All records received.
1266248518 2018-08-28 Preliminary Restoration No Violations Preliminary Restoration found the area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The well pad and lease road are of stone surface construction and in good repair. 4 total well are now present on the well pad. The said well is equipped with a lower A and upper B section well head assembly with 2 diameter riser pipes attached to each well head section, the riser pipes are equipped with valves and gauges to monitor annular pressure. The A section gauge which monitors the pressure between the 13.375 and 9.625 diameter casing string reads 600 pounds pressure. The B section gauge which reads the pressure between the 5.5 and 9.625 diameter casing string reads 1,250 pounds pressure. The 5.5 internal casing pressure is 796 pounds and the flow line pressure downstream of the well head choke is 352 pounds. The tubing head pressure is 395 pounds pressure. On the left rear side of the well pad is the production fluid facility consisting of (3) steel construction 400 barrel capacity tanks along with a single 100 barrel tank, all situated inside a polymer lined steel containment dike area. There is a 3rd party midstream pipeline point and compressor on a separate pad at the rear of the location.
1272210946 2015-02-10 Preliminary Restoration No Violations Status check inspection on the Preliminary Restoration found the well in production with 2.875 tubing set in the well bore and proper well identification posted on the well head. There is 3 -400 barrel capacity production fluid storage tanks on location along with a 100 barrel tank, all located in a metal dike area with a plastic type containment dike. There is 2 gas processing units along with 2 mechanical separators located on the far East side of the well pad to support both wells on the pad now producing. Proper well identification is posted at both the well head and tank battery. The Preliminary Restoration was passed on this day. No violations noted.
1380971328 2013-10-05 Casing String Witnessed No Violations On this day I was on location to witness the cementing of the surface casing, 420 of 24 surface was set inside a 425 x 28 borehole and cemented by Universal well services to surface.
1381253306 2013-10-08 Drill / Deepen / Reopen No Violations Status check on the drilling progress found the current drilling depth to be 668 using a 22 hammer bit and 7813 cfm of air with a directional survey being conducted every 43, The permitted depth for this well bore section is 864 and is considered a mine string. I reviewed the possibility of breaking down the coal seam pillar and offered to review some of the prior mine void cement jobs with the company if they would like.
1382012049 2017-03-01 Work Over Reconditioning No Violations On site inspection to witness the 5.5 production casing remediation process with Nine Energy Wireline Service and Deep Well Service workover rig. The production tubing has been removed from the well bore and a completion well stack installed consisting of a master control valve along with a hydraulic blind and pipe rams for the tubing string and the wireline well head control stack. 100 barrels of fresh water was pumped down the 5.5 in attempt to load the hole with the well not loading and on vacuum. Nine Energy ran a 4.5 gauge ring and junk basket down to 10,499 prior to running and setting a cast iron bridge plug at 11,025. The plug was then pressure tested and the 5.5 perforated at 10,950 with no circulation being established, a second set of perforations was shot at 10,800 with circulation being achieved at that point.

1388069792 2013-12-26 Drill / Deepen / Reopen No Violations Status check inspection to document the recent setting of a kick plug over a lost drill bit assembly, On 12/21/2013 Universal well services cement services set a 417 sack plug with a 17#- 1.04 yeild-0.5% cr-3 mix from 7510 to 6968. On 12/26/13 my inspection found the current drilling depth to be 6998 and the company time drilling with the plan to drill a 5 degree kick off of the plug. The company is drilling on oil based drilling mud.
1391011425 2014-01-29 Casing String Witnessed No Violations Status check inspection on the running and cementing of the 9.625 2nd intermediate casing, 8,568 was the total depth drilled with a 12.250 diameter, 8,542 of 43.50# P-110 casing was ran and cemented by Schlumberger cement services using a lead and tail cement mix with a estimated top of cement of 200 from surface.
1392988539 2014-02-20 Drill / Deepen / Reopen Violations Noted On the day of my inspection I was asked by Monroe County Inspector Bill Stolarik to pick up and input production casing cement information while inspecting 3411124160000 Shroyer Unit 2H. While inputting information into RBDMS, I found that the cement ticket was not accurately filled out and returned to the Company Mans office. Requesting information missing from the cement ticket I was informed that the cement job locked up and approximately the cement was at 5200 feet inside the 5.5 inch casing. The County Inspector was not notified that the job was not successful and the State will need to see proof of top of cement prior to stimulation of well and if remedial cementing is needed. Monroe County Inspector Bill Stolarik is aware and will follow up prior to the completion of the well.
1395671058 2014-03-24 Completion Testing No Violations Notification was received from Kyle Bradford 740-525-9498, who is the completion supervisor with Eclipse Resources, Kyle informed me that the well had annular production casing pressure is exceeding 1800 psi and a 3rd party contract company is on site controlling the pressure through a chock valve assembly and flair stack, the Division has been made aware that during the production casing cement job the cement truck power unit broke down part way into displacement and there is cement left inside the production casing to be drilled out and possibly no correct cement coverage on the production casing annular side. The company plans to bring in a service rig and drill the cement out of the casing and bond log the well to determine the top of cement on the annular side, Kyle was informed that the Division management would have to be involved with the finial evaluation and decision.
1396466371 2014-04-02 Work Over Reconditioning No Violations Status check inspection of the work over process to remove the cement that was left inside the production casing during the production casing cement job, mercer well services work over rig is on the hole and drilling out the cement at a current depth of 7,454, they initially set down on cement at 5,018, the company man will call me for the bond log when they get all the cement drilled out.
1396911611 2014-04-07 Work Over Reconditioning No Violations Notification was received on this day from Bruce Carpenter with Eclipse Resources Environmental Division that a earthen slip had occurred on the South side of the Shroyer well pad during the last 48 hours, I responded and met with Diversified Engineering employees that were surveying the impact site along with Bruce Carpenter with Eclipse Resources and Force construction company who will be doing the repair work, Diversifieds engineers measured the total impact area affected by the slip and calculated a 200x 200 or approximately 1 acre slip that has occurred with materials breaching the origional limits of disturbance, at this time Eclipse has released the workover rig and removed all equipment off location and is bring in a core boring rig to determine the subsurface foundation and shall submit a design repair plan based on these findings.
1404107754 2014-06-29 Completion Testing No Violations Status check inspection to witness the initial break down pressure and monitor the annular pressure. Initially before starting the procedure the backside of the 5.5 production casing showed 1600 psi and the pressure was bled off two different times to 1000 psi and 800 psi and upon the closing of the choke valve the pressure immediately built back up to 1600 psi. The casing was then loaded and a 2.9 barrel per minute rate established with the top pressure reached during the test was 10,868 psi with the annular pressure climbing to 1,940 psi, the formation broke at 7,704 psi and the annular pressure dropped to 1,844 psi also. 40 perforations were shot in 4-10 shot bursts between 18,495 to 18,345 and the company was experiencing difficulties establishing rate due to casing pressure increasing internally possible due to debris inside the casing from the cement that was drilled out, the company me shall post me if any changes occur.

1488209727 2017-02-27 Work Over Reconditioning No Violations Status Check inspection on the 5.5 production casing cement squeeze job found Deep Well Services workover rig on the hole rigging up and preparing to pull the production tubing string. The well bore has been equalized hydrostatically with brine water. The rig will pull the production string today and a wireline truck is slated to be on location on 02/28/17 to set a bridge plug and perforated the production string above the top of cement on the annular side in order to squeeze cement into the 5.5 production casing annular space. A proposed plan has been received by the Division.
1488465665 2017-03-02 Work Over Reconditioning No Violations Status check inspection on the 5.5 diameter production casing cement squeeze job. On 02/28/2017 Nine Energys wireline service set a 10,000 psi rated cast iron bridge plug at 11,025, the plug was pressure tested to 7,500 psi. Nine Energy then shot 9 perforations at 10,800 with the well circulating at that point and 600 barrels of fresh water was circulated through the well bore. 03/01/2017 A cement retainer was ran on 2.875 diameter tubing to a measured depth 8,919 by Deep Well Services. 03/02/2017 Schlumberger Cement Service squeezed the 5.5 annular space through the 2.875 by pumping 60 barrels of 12.5 ppg of Mud push pre sweep followed by a 247 barrel lead slurry consisting of 1075 sacks of TXI cement followed by a 42 barrel cement mix consisting of 200 sacks of class A cement with the cement displaced with 49 barrels of fresh water and the hole cleaned up by lifting the tubing the tubing string out of the cement retainer and reverse circulating the well bore with 100 barrels of fresh water.

Estimating tomorrow for running 5.5 inch 20#/ft. P-110, production casing upon completion Schlumberger will cement casing. Nomac Drilling Rig #359 will then skid to 3411144160000 Shroyer Unit 2-H. Site inspection revealed no violations.
