All drilling equipment and debris has been removed and the soil has been graded out to the approximate original contour. The well is equipped with a plunger lift system and connects to a tank battery that consists of one 140 barrel stock tank, one 50 barrel poly brine storage tank, one separator and an earthen dike. The stock tank is equipped with a flash arrestor and a charcoal filter. Proper identification was posted and I found no leaks or spills. No violations at this time. This well site passes preliminary restoration. |

| 1553261613 | 2019-03-22 | Production Wells | Violations Noted | I was on location at the request of DOGRM Emergency Response to investigate the report of staining on a tank and the presence of a green, algae like substance on the ground around the tank. My inspection found the tank battery diked and containing one (1) 100 barrel tank, one (1) 50 barrel poly tank, and a vertical separator with an associated meter run. There is old staining on the 50 barrel poly tank from the- hatch lid opening down to just above the gravel in the tank battery. The hatch lid is partially open and there is a one (1) inch line plumbed through the center of the lid. The staining was dry and I found no oil on the ground. Both of the tanks are currently empty. The green substance is algal material that has formed along the east and south sides of the secondary containment area where water collects during precipitation events. I found the same algal material in the standing water in the field between the wellhead and tank battery. The wellhead consisted of 4-1/2 production casing and 1-1/2 tubing equipped with a plunger lift system. A gauge indicates 240psi on the tubing. It is determined that there is no release at this tank battery. The only issue I found was that there was no ID signage posted as required in violation of 1501:9-9-05(A)10 of the Ohio Administrative Code. I called Supervisor Jim Paumier and requested that the proper ID sign be posted as soon as possible. I will follow-up in one (1) week. |
