Columbia Gas Transmission has sent me a plugging plan that advises the Sidney #1 well will be plugged by 10/1/2019. I took two photos of the well location during the inspection. 1554125604 2019-04-01 Production Wells No Violations I arrived to this well at 8:20 AM to conduct an inspection on this well in regards to a phone call I received from Columbia Gas Transmission Engineer Devin Holter on 3/29/2019. Dan Talbot with Columbia Gas Transmission was present when I arrived. My inspection revealed that a sinkhole has opened up in a field within 140 feet of the location of this API permit number. The sinkhole was approximately 5 feet in diameter at ground level. The sinkhole tapered down to approximately 12 inches a few feet below ground level. I did not observe any casing in the hole. Dan Talbot ran a tape line down the well and reached approximately 200 feet in depth. CGT Engineer Devin Holter arrived during the inspection and showed me a record of the well. The well record did not indicate that the well was plugged properly with clay or cement. I advised Dan Talbot and Devin Holter during the inspection that the well shall be plugged according to OAC 1501:9-11. Dan Talbot informed me that they would barricade the well and place a steel plate over the hole until the well can be plugged. The well owner/operator shall submit a written plugging schedule to Brandon Mitchell at 5880 Memory Road Zanesville, Ohio 43701-9553 within 30 days, per ORC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. Time of inspection: 8:20 AM. Weather: 31 degrees F, Sunny.
