0105668171 2010-12-20 Plug / Plug Back No Violations Witnessed plugging and abandonment. Pumped cement inside 3.5 csg from 1790 to 1000 for Lime plug. Perforated at 770 (6 shots) and set 3.5 cement retainer at 753 then perforated again at 300 (6 shots). Stabbed into retainer with stinger. Pump pressure reached 1250lb w/no estbl circulation and unable to squeeze w/ stinger. Therefore, we would now rely on hydrostatic pressure and stabbed-out of retainer to pump 4bbl of cement to 315. Pulled 15 joints. Closed return line on BOP w/initial circulation and pumped 0.75 bbl cement w/pressure of over 800 lbs. Opened line and observed cement returns. TOH. Cement dropped and unable to visually see. Topped off to surface. The above procedures were done with the idea that at least hydrostatic would assist with squeezing of cement on backside of casing to seal Berea formation. Background: Set a 600 bottom hole plug on 12/15/2010, through 2.063 tubing (inside 3.5 casing cemented to surface). Note: Columbia records had showed 3.5 csg to surface inside un-cemented 5.5 csg; and 7 csg left from 534-782 w/lead BH packer also not cemented and placed across the Berea. In the 1960s, the 5.5csg was perforated and cemented from 210 to surface. Recent new drill for Columbia, Permit No. 3596 showed freshwater at 405. No casing would be retrieved from this plug job.
