0064686979 2016-02-26 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. I found that all the wells have been drilled and night caps are placed on the wells bores. A gas sales line has been ran to the location but was not connected. No production equipment was present on the day of my inspection.

1107215674 2015-08-29 Casing String Witnessed No Violations The rig ran 500 of 13-3/8 61# R-3 casing and the required float equipment to 497. Schlumberger cemented it with 465 sacks of Class A cement. 26 bbls of good cement was returned to the surface. The plug was landed at 203 psig and bumped to 739 psig with 13 psig of fall off. The pressure was released and the floats held.

1936951953 2015-06-13 Casing String Witnessed No Violations I an on site today to witness the running and the cementing of the 20 inch diameter casing string. Express Energy Service drilled a 26 inch diameter hole to 120 ft. and set 120 ft. of 94 lb. casing. Express Energy Service cemented with 230 sacks of class A cement at 15.6 lb. and a yield of 1.18 and had good cement returns to surface-14 bbl.
1942314390 2018-04-10 Preliminary Restoration No Violations On the day of my inspection I found this pad containing 8 properly labeled wells that are producing. This well has a plunger lift system wellhead. Pressure gauges read as follows: Surface 1 psi, Intermediate 1 psi, Production 531 psi, and Tubing 224 psi. Casing hanger is plumbed into buried flow line. Production equipment includes fifteen 400 barrel steel tanks within their own steel containment, three compressors, three line heaters, eight sand separators, eight GPUs, a vapor recovery stack, and two combustors. One heater treater onsite. Site is stable and vegetated. Site meets preliminary restoration requirements of 1509.072A.
