0097561102 2018-01-30 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Precision Rathole was drilling at 54 feet with a planned total depth of 120 feet. Upon reaching total depth Precision will run 120 feet of 24 inch diameter 94.62 pounds per foot conductor pipe with cementing to follow.
0221050845 2018-01-29 Casing String Witnessed No Violations On the day of my inspection after receiving a call from Eclipse Consultant Charlie Frazier concerning the wellbore caving in after spudding the well on 1/27/18, I witnessed that below the cellar the sand based material had collapsed into the wellbore with drilling ceased. A meeting with Mr. Frazier and Cement Specialist Lex Cook was held where they propose to clean the wellbore up with a 42 inch diameter bit and install 36 inch drive pipe into bedrock and remove any excess water and then utilizing 9-grout fill the backside to surface. Precision cleaned the wellbore up with a 42 inch diameter bit to 16 feet and ran 21 feet of 36 inch 195.175 pound per foot drive pipe and drove it to 26 feet leaving it set at 5 feet below surface. Water was removed from the annulus and Caldwell Redi-Mix pumped 10 yards of 9-sack grout bringing grout to surface. Tomorrow Precision Rathole will commence drilling a 30 inch diameter hole to the total depth of 120 feet and then running 24 inch diameter conductor pipe with cement to follow.
0405209031 2018-09-10 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. During my inspection Deep Well Services stand alone snubbing unit 10 was on location drilling out frac plugs on 3405924521 and production equipment was being assembled. The area affected by pad construction has all been remediated with vegetation growth. The lease road was paved with asphalt and concrete and the pad was gravel. The location meets the preliminary restoration requirements for 1509.072 (A).
0564713284 2018-02-11 Casing String Witnessed No Violations On the day inspection I witnessed that Yost Drilling Rig #5 had drilled a 17.5 inch diameter hole to the depth of 1214 feet. Wyoming Casing Crew ran 1216.56 feet of 13.375 inch 54.5#/ft. J-55 casing set at 6.5 feet above KB. KB height is 12 feet above ground level. Schlumberger was on site and loaded their lines with 5 barrels of fresh water and performed a low pressure test at 1000 pound per square inch and a high pressure test at 3000 pounds per square inch. Schlumberger gained circulation with 181.4 barrels of fresh water, 20 barrels of gel and 10 barrels of fresh water. Schlumberger cemented casing with 1040 sacks of Class A cement 15.6 pounds per gallon, 1.19 cubic foot per sack cement with 1% S001 CaCl, .1 pound per sack D130 Flake, .2% D202 Dispersant, .020 gallon per sack D047 Antifoam and .5% B348 FLAC. Cement weight was verified by pressurized mud scales and displaced with 181.4 barrels of fresh water. Cement was circulated to surface with good returns of 30 barrels. Plug was landed at 510 pounds per square inch and pressured to 1080 pounds per square inch for 5 minutes and the floats held with 1 barrel back. Mix water was tested revealing chlorides of 60 parts per million, pH 6 and 60 degrees.
0803123742 2019-03-12 Production Wells No Violations I was accompanied by ODNR Southeast Regional Manager Brent Bear and the purpose of our inspection was a status check. During my inspection the well was equipped with a plunger lift system with valves in the open position. The 13.375-inch diameter surface casing had 0 pounds per square inch, the 9.625-inch diameter intermediate casing had 0 pounds per square inch, the 5.5-inch diameter production casing had 205 pounds per square inch, and the 2.875-inch diameter tubing had 800 pounds per square inch. The production equipment consisted of four sand traps, four gas production units, and two 400-barrel tanks. The produced water was being separated on pad and being stored in the 400-barrel tanks. Produced oil and gas was being gathered at the Eclipse outlaw pad via pipeline. Identification was located at the wellhead.
0994846541 2018-07-13 Completion Testing No Violations The purpose of my inspection was to witness stage 1 of hydraulic stimulation operations on the 1H. During my inspection I witnessed Halliburton utilize sand boxes and an above ground storage tank to pump a total of 270,020 pounds of sand proppant and 321308 gallons of water. The average rate was 82.5 barrels per minute, breakdown pressure was 5012 pounds per square inch, and the treatment pressure was 9113 pounds per square inch. They were completing through a 5.5 inch diameter production casing and annular pressure monitoring was present and displayed in the Frac Van.

During my inspection the well was equipped with a production tree style wellhead with valves in the open position. The production equipment consisted of four sand traps, 4 gas production units, and two 400-barrel tanks. Identification was posted at the wellhead. Southern Fencing company was on location installing chain link fencing around the pad.

The 13.375-inch diameter surface casing had 0 pounds per square inch, the 9.625-inch diameter intermediate casing had 0 pounds per square inch, the 5.5-inch diameter production casing had 250 pounds per square inch, and the 2.875-inch diameter tubing had 900 pounds per square inch. The production equipment consisted of four sand traps, four gas production units, four frac tanks, and two 400-barrel tanks. The produced water was being separated on pad and distributed in the frac and 400-barrel tanks. The oil and gas were being gathered at the Eclipse outlaw pad via pipeline. Identification was located at the wellhead.

Stingray Pressure Pumping was onsite and pumped 2 barrels of fresh water, performed a 300 pounds per square inch pressure test and cemented pipe with 350 sacks Class A cement with 2% CaCl and .25 pound per sack of Celluseal weighing 15.6 pounds per gallon and a yield of 1.2 cubic foot per sack. Cement was weight was verified with pressurized mud scales and displaced to 97 feet with 51 barrels of fresh water with good cement returns of 20 barrels to surface. Mix water was tested revealing chlorides of 32 parts per million, pH 7 and 58 degrees.
