0212186130 2018-10-09 Preliminary Restoration No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. The production equipment consisted of four sand traps, 4 gas production units, and two 400-barrel tanks. Identification was posted at the wellhead. Southern Fencing company was on location installing chain link fencing around the pad. Location meets requirements for preliminary restoration.
0500703194 2018-01-29 Casing String Witnessed No Violations On the day of my inspection after receiving a call from Eclipse Consultant Charlie Frazier concerning the wellbore caving in after spudding the well on 1/27/18, I witnessed the below the cellar the sand based material had collapsed into the wellbore with drilling ceased. A meeting with Mr. Frazier and Cement Specialist Lex Cook was held where they propose to clean the wellbore up with a 42 inch diameter bit and install 36 inch drive pipe into bedrock and remove any excess water and then utilizing 9-grout fill the backside to surface. Precision cleaned the wellbore up with a 42 inch diameter bit to 16 feet and ran 21 feet of 36 inch 195.175 pound per foot drive pipe and drove it leaving it set at 5 feet below surface. Water was removed from the annulus and Caldwell Redi-Mix pumped 10 yards of 9-sack grout bringing grout to surface. Tomorrow Precision Rathole will commence drilling a 30 inch diameter hole to the total depth of 120 feet and then running 24 inch diameter conductor pipe with cement to follow.
0709172815 2019-03-12 Production Wells No Violations I was accompanied by ODNR Southeast Regional Manager Brent Bear and the purpose of our inspection was a status check. During my inspection the well was equipped with a plunger lift system with valves in the open position. The 13.375-inch diameter surface casing had 0 pounds per square inch, the 9.625-inch diameter intermediate casing had 1290 pounds per square inch, the 5.5-inch diameter production casing had 461 pounds per square inch, and the 2.875-inch diameter tubing had 900 pounds per square inch. The production equipment consisted of four sand traps, four gas production units, and two 400-barrel tanks. The produced water was being separated on pad and being stored in the 400-barrel tanks. Produced oil and gas was being gathered at the Eclipse outlaw pad via pipeline. Identification was located at the wellhead.

The Eclipse company man estimates that the cement job will be Wednesday 5/2/18.

The pad is named Jupiter and on this date the pad consist of two production wells 3405924520 and 3405924521. During my inspection the well was equipped with a plunger lift system with valves in the open position. The 13.375-inch diameter surface casing had 0 pounds per square inch, the 9.625-inch diameter intermediate casing had 1100 pounds per square inch, the 5.5-inch diameter production casing had 530 pounds per square inch, and the 2.875-inch diameter tubing had 1100 pounds per square inch. The production equipment consisted of four sand traps, four gas production units, four frac tanks, and two 400-barrel tanks. The produced water was being separated on pad and distributed in the frac and 400-barrel tanks. The oil and gas were being gathered at the Eclipse outlaw pad via pipeline. Identification was located at the wellhead.

1754808326 2018-09-10 Completion Testing No Violations The purpose of my inspection was a status check on a preliminary restoration. During my inspection Deep Well Services stand alone snubbing unit 10 was on location drilling out frac plugs on 3405924521 and production equipment was being assembled. The area affected by pad construction has all been remediated with vegetation growth. The lease road was paved with asphalt and concrete and the pad was gravel. The location meets the preliminary restoration requirements for 1509.072 (A).
2046745238 2018-02-01 Casing String Witnessed No Violations On the day inspection I witnessed that Precision Rathole had drilled a 30 inch diameter hole to the depth of 100 feet and ran 102 feet of 24 inch diameter 94.62 pounds per foot conductor pipe with a 2 foot stick up. Stingray Pressure Pumping was onsite and pumped 2 barrels of fresh water, performed a 300 pounds per square inch pressure test and cemented pipe with 290 sacks Class A cement with 2% CaCl and .25 pound per sack of Celluseal weighing 15.6 pounds per gallon and a yield of 1.2 cubic foot per sack. Cement was weight was verified with pressurized mud scales and displaced to 92 feet with 48.3 barrels of fresh water with good cement returns of 12 barrels to surface. Mix water was tested revealing chlorides of 32 parts per million, pH 7 and 57 degrees.
