I met with Eclipse Consultants Ryan Bartholomew and Matt Amos who stated that rig mats and containment will be laid today. Precision Rathole will be onsite tomorrow with in preparation of spudding of the well this week.
0847540944 2019-01-31 Preliminary Restoration No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a plunger lift system with valves in the open position on the production tubing. Two production flow lines are attached to the well, one to produce from the tubing and one to produce from the production casing. The 13.375-inch diameter surface casing had 0 pounds per square inch, the 9.625-inch diameter intermediate casing had 0 pounds per square inch, the 5.5-inch diameter production casing had 600 pounds per square inch, and the 2.875-inch diameter tubing had 443 pounds per square inch. The production equipment consisted of eight sand traps, eight gas production units, and three 400-barrel tanks. The produced water was being separated on pad and distributed in the frac and 400-barrel tanks. The oil and gas were being gathered at the Eclipse Hayes CGF pad via pipeline. Identification was located at the wellhead.

Flowback operations will commence later this month after completing operations on the 34059244140000 Fineran 2H.
1490027208 2017-03-20 Casing String Witnessed No Violations On the day inspection I witnessed that Precision Rathole had drilled a 30 inch diameter hole to the depth of 120 feet, hitting water at 60 feet and ran 120 feet of 24 inch diameter 94.62 pounds per foot conductor pipe. Stingray Pressure Pumping was onsite and pumped 2 barrels of fresh water, performed a 500 pounds per square inch pressure test, gained circulation with 8 barrels of fresh water and cemented pipe with 290 sacks Class A cement with 2% CaCl and .25 pound per sack of Celluseal weighing 15.6 pounds per gallon and a yield of 1.2 cubic foot per sack. Cement was weight was verified with pressurized mud scales and displaced to 110 feet with 57.75 barrels of fresh water with good cement returns of 13 barrels to surface. Mix water was tested revealing chlorides of 46 parts per million, pH 7 and 53 degrees.
1490102606 2017-03-21 Drill / Deepen / Reopen No Violations On the day inspection I witnessed cement remained at the surface and Precision Rathole was preparing to drill the mouse hole. Grant Smith Contractors were onsite rigging up air packs on the north side of the well pad.
1495555410 2017-05-23 Drill / Deepen / Reopen No Violations Status check inspection on the well drilling process found H&P Drilling rig 531 on location and preparing to begin running in the hole with the 8.5 diameter Schlumberger Bottom hole directional drilling assembly in preparation to drill the horizontal curve and lateral well bore section. The 9.625 intermediate casing string was recently set at a measured depth of 7,030. The drilling rig and support equipment is placed on polymer containment and the well pad earthen perimeter berm is serving as the primary containment with the drain valves in the closed position.
1680791408 2017-05-18 Casing String Witnessed No Violations On the day of my inspection I witnessed that H&P Flex Rig #531 had drilled a 17.5 inch diameter hole to the depth of 1376 feet and hitting water at 1100 feet with a flow of 200 barrels per hour for 1 hour and then stopping. 1857695056 2017-06-27 Completion Testing No Violations On the day of my inspection I met with Eclipse Consultant Dustin Moore and reviewed upcoming frac schedule which will consist Halliburton fracing 62 stages beginning upon the completion of the 34059244950000 Outlaw C 11H and the 34059244940000 Great Scott 3H. The well will be zipper fraced with the 34059244970000 Yost B 5H. I completed walk around confirming at this time containment is being completed for the installation of the onsite produced water tanks. Upon completion tanks will be set and offloading of sand will begin with commencement of frac to follow on the Outlaw and the Great Scott.

2027419940 2018-07-19 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. During my inspection Deep Well Services Rig 4 was rigged up on the Yost B5H. I spoke with Eclipse Company man and he stated that they were in the process of installing a plunger lift system. Two 500-barrel frac tanks and one 500-barrel gas buster were being utilized. All casing strings were equipped with a pressure gauge and were being monitored. The area effected by the construction of the pad has been restored with vegetation established. The location meets preliminary restoration requirements listed under ORC 1509.072(A).
